Responsibilities

Clinical

  • Performs restorative services according to physician's orders and/or nursing assessments as reflected in the resident's plane of care.

  • Assists residents to achieve and maintain a optimal level of self-care and independence.

  • Uses equipment correctly and safely in performing restorative care.

  • Follows infection control technique in performing restorative care.

  • Observes and reports signs and symptoms of change in condition and resident response to treatment.

  • Participates in shift report according to company and hospital requirements.

  • Performs routine restoration and preventative nursing procedures, i.e., ROM, transfers, passive/active exercises, feeding program, weights).

  • Participates in formulation, implementation and revision of the resident plan of care.

  • Instructs and encourages, as needed, residents and families in goals and procedures or restorative services.

  • Applies and uses adaptive equipment and supplies, i.e., splints, pillows, walkers) safely and according to policy and procedure.

  • Makes routine rounds throughout the unit to observe the appropriate use of restorative equipment and supplies.

  • Follows all infection control guidelines during the performance of restorative care (i.e., hand-washing, universal precautions, linen handling).

  • Makes frequent observations for indication of resident's condition during performance of restorative care.

Administrative

  • Maintains all documentation as required by Federal, State and company regulations.

  • Exchanges all information necessary for quality resident care.

  • Organizes resident care activities to complete resident care based on priority of resident needs.

  • Documents in resident health care record according to policy and procedure.

  • Reports promptly any changes in resident's condition or other concerns to the Charge Nurse.

  • Reports inappropriate use of restorative equipment and supplies to the Director/Head Nurse.

  • Obtains specific instructions for ongoing restorative care during and at the conclusion of specialized therapy services.

  • Organizes workload to complete treatments ordered. Reports all workload conflicts to Director/Head Nurse.

  • Notifies Director/Head Nurse of supply and equipment needs.

  • Follows established hospital and department policies.

  • No more than 2 written substantiated complaints representing a breach of policy, procedure or professional behavior since last evaluation.

  • Demonstrates the philosophy of team concept.

  • Participates in group projects and staff meetings with suggestions that enhance the work environment and increase productivity.

  • Communicates well with supervisor, reporting problems with equipment, supplies or procedures. Requests assistance as appropriate.

  • Maintains confidentiality as related to job responsibilities.

  • Exhibits willingness to resolve problems as they arise.

  • Consistently projects positive professional image through appearance and behavior.

  • Attends 75% of staff meetings or reads and initials minutes.

  • Completes assigned work within shift.

Guest/Interdepartmental Relations

  • As observed by representatives of management, all interactions are conducted in a professional manner.

  • Verbal and written feedback resident's family members/significant others, medical staff, visitors and co=workers indicates behavior conducive to positive guest relations.

  • Consistently exhibits appropriate phone protocol (e.g.,, answers phones promptly, is courteous and helpful).

  • Consistently contributes to the team effort (e.f., assists co=workers when need is observed or upon request, offers constructive suggestions rather than complaints).

  • Consistently displays cheerful and positive attitude.

Professional Growth and Development

  • Completes annual updates within established time frames.

  • Completes general and departmental orientation within established time frames.

  • Attends 75% of in-services.

  • Maintains certification as appropriate.

  • Manages workload to avoid overtime.

  • Consistently arrives on time.

  • Does not exceed sick time as established by hospital policy.

  • Maintains professional attire, including visible ID.

  • Complies with all safety and infection control standards.

Performance Improvement

  • Consistently strives to understand, anticipate and meet the needs, expectations and satisfaction levels of residents and other "customers."

  • Errors, inefficiencies and inaccuracies are brought to management's attention, with suggestions for improvement.

  • Demonstrates knowledge of each step of the PI process and participates in the formation and implementation of the PI plan.

  • Identifies and implements methods of controlling costs or generating revenue, while providing maximum value to both the resident/customer and the hospital.

  • Maximizes efficiency in all departmental operations.

  • Measures progress against quality goals.

Qualifications

Minimum Education

  • High School Graduate Minimum Experience

  • 1 year prior acute care experience; 1 year long-term care experience as CNA and 6 months Medical/Surgical experience preferred. Required Certification/Registration

  • Current CNA certificate; RNA training/certificate; BLS; proficiency in both written and oral English.
