Provides clinical supervision to rehab personnel and other assigned rehab support personnel providing care to home care patients, under the direction of the Director of Rehab and in collaboration with the Administrator of Home Care.
This is a full time position to work for home health, 50% in the field and 50% administrative, supporting our East of I-77 service area.
Provides direction and support to the home care field staff.
Supervises Starts of Care, resumptions of care and routine visits.
Conducts assessment of patient's needs and initiates rehab care plans.
Evaluates and documents patient progress.
Facilitates the planning, implementation, development, and evaluation of rehab programs, initiatives and objectives for the geographic area.
Assists with the development and implementation of policies and procedures to insure efficient and effective delivery of health services in a home care.
Serves as a liaison between the Manager, caregivers, and other areas within the organization.
Identifies and resolves issues affecting the delivery of patient care services in the homecare setting.
Oversees rehab personnel and patient care providers to ensure quality of care.
Anticipates and resolves patient care issues.
Ensures continuity of patient care by actively participating in and facilitating communication and collaboration among and between department staff and other departments re operational issues.
Reviews major operational patient care issues with leadership.
Provides ongoing clinical evaluation of therapists and other direct and indirect patient care providers on assigned area.
Counsels employees on performance issues, administers initial corrective actions, collaborates in monitoring attendance issues.
Alerts leadership of significant clinical or other performance issues.
Participates in interviewing, selecting and appraising the performance of employees.
Administers Center, departmental and CC policies and procedures.
May prepare schedule of work assignment assuring adequate coverage on all shifts.
Participates in and supports quality improvement projects, outcomes studies and research initiatives; participates in department and committees as assigned.
Assists in orienting new personnel, continuing education and inservice educational programs.
Other duties as assigned.
Bachelor's Degree in Occupational Therapy, Physical Therapy or a Master's Degree in Speech Language Pathology.
Strong demonstrated clinical knowledge and skills; good oral, written and interpersonal skills, strong analytical and organization skills.
Must have an active current State license in one of the following disciplines: Physical Therapist (PT), Occupational Therapist (OT) or Speech Language Pathologist (SP).
Basic Life Support (BLS) through the American Heart Association (AHA) required.
Valid Ohio Driver's license upon hire; per policy 8-175 thereafter.
Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter.

Within scope of job, requires critical thinking, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment.
Minimum of three year's clinical experience.
Minimum two years clinical experience in a home health setting preferred.
Previous supervisory experience desirable but not mandatory.
Must be able to travel to patient's home.
Must be able to climb stairs, pull, push and facilitate movement of the patient.
Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move more than 100 pounds.
Must be able to transport laptop computer and/or necessary medical/patient equipment and supplies.
Must be able to read, write and communicate with patients.
Must have normal or correctable vision.
Must have normal or correctable hearing to be able to hear normal conversations.
