Essential Duties

1.Performs physical assessments of patients, including triage duties and advises on treatment or course of action by having a thorough knowledge of nursing care standards and principles. 2. Identifies and evaluates nursing services problems and develops and implements appropriate programs and services, including assessing staffing needs and arranging coverage as needed. 3. Develops, implements, and manages nursing care plans in collaboration with physicians. 4. Instructs patients and families in proper treatment and advises patients on health maintenance and disease prevention. 5. Provides leadership, guidance, direction to clinical staff, and collaborates with nursing at all levels. 6. Determines standards for quality assurance and infection control procedures; trains staff in universal and isolation precautions in the area of infection control. 7. Performs diagnostic and therapeutic measures in collaboration with physicians. 8. Evaluates the quality and effectiveness of nursing practice and nursing services. 9. Records patients' medical histories and symptoms, performs diagnostic tests and analyzes results, operates medical machinery, administers treatment and medications, and coordinates with patient follow-up and rehabilitation; assures all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines.

Required Education and Experience

Bachelor's Degree in Nursing. Two years experience or an equivalent combination of education and experience.

Position Focus:

In 1971, Yale University established Yale Health to provide health services to its facility, staff and students through a multidisciplinary health maintenance organization located on campus. Yale Health has more than 47,000 members including students, staff, faculty and their families who come from every state in the country and almost every country in the world. With over 50 years of service to the Yale community, our state-of-the-art facility at 55 Lock Street, is where our members receive most of their care from our 150+ providers. It is a 144,000 square foot medical facility with over 90 exam rooms, an Acute Care Department, a 15-bed inpatient facility with two negative pressure rooms, a diagnostic imaging suite including MR and CT scan, x-ray and ultrasound and a full-service retail pharmacy.

Under the direction of the Clinical Manager of Pediatrics, provide direct nursing care as well as help to oversee the daily operations of the department. Assist in coordinating high level clinical and administrative functions in support of the care team at Yale University Health Services. In the absence of the Clinical Manager of the Department, this role will be responsible for operations and decisions to ensure smooth departmental operations that meet the needs of our patients.

Additional responsibilities include:

  • Supports the pediatric care team by assuming responsibility for and coordinating all facets of nursing support for pediatric members and their families. Coordinates and/or delivers direct nursing care to all members of the pediatric family unit along the continuum of health, starting from wellness to illness.

  • Clinical judgment and decision making in accordance with clinical practice guidelines, protocols, and clinician orders, including follow up, preventative health, maintenance, teaching and education. Assume responsibility for evaluation and disposition of scheduled patients as well as same day- walk in patients. Care includes teaching and education of family members as appropriate.

  • Significant emphasis on telephone nursing/ triage, as well as case management and care coordination. Will function as a liaison to clinicians regarding triage/clinical care decisions and facilitate patient flow. Anticipate patient care needs based on patient assessment and prioritize care. Foster ongoing improvement of access to services.

  • Nursing responsibilities will include Telephone Triage RN role, Nurse Coordinator RN role, and Immunization RN role. In addition, Acute Care RN role may be required on occasion.

  • Assure appropriate documentation of patient encounters, including patient teaching, self care advice, and coordination of health services as indicated.

  • Oversight and supervision of support staff on a care team and the ability to delegate to clinical and support personnel, within their scope of practice/responsibility. This includes patient care assignments, additional project work and coverage as needed.

  • Identify priorities, provide training and evaluation of support staff as needed.

  • Identify education needed to ensure competent delivery of patient care for oneself and others in the department.

  • Responsible for performance evaluations, training, coaching and orientation of members on a care team.

  • Participate with the Pediatric Leadership in creating and monitoring departmental quality improvement activities.

  • Will assume charge nurse responsibilities.

  • Communicate with all health care clinicians to ensure appropriate on-going care.

  • Responsible for maintenance of a comfortable, orderly, safe and clean environment.

  • Additional projects and responsibilities as indicated, including outreach and population health initiatives.

  • Participate in YUHS committees, programs, teams and activities as needed.

  • Performs and/or supervises all CLIA waived testing performed in department(s).

Preferred Education, Experience and Skills:

Supervisory experience; evidence of maintenance of professional skills and competencies by reading, attending professional in- services and educational events. Familiar with Joint Commission accreditation standards. Experience with electronic medical records.
