Access Services is looking to welcome an Assistant Coordinator to our Lifesharing program. The home base for this role is at our main office in Fort Washington, PA and requires regular travel in the surrounding Delaware Valley region.
You will be part of a team working to support adults with intellectual disabilities/Autism who live in our Lifesharing provider host homes. Our team works with the provider families and the individuals to ensure the home meets the needs and preferences of the person served. As an Assistant Coordinator you support our team of Coordinators, carry a reduced caseload, and transport individuals that live in Lifesharing homes.

A week in the life of a Lifesharing Coordinator might look like:
Essential Functions
1. Assist with the completion or coordination of a comprehensive assessment for each individual being referred.
2. Assist with the development, implementation, updating and review of individual support plans that are strengths based and person centered in collaboration with the team and the
individuals being served.
3. Assist with the coordination of the implementation of all supports outlined in each individual’s plan.
4. Assist or facilitate regular team meetings for the planning and review of services, as required.
5. Meet with the individuals being served and their families, at a minimum quarterly, to evaluate service needs.
6. Collect and prepare program and/or individual related data as requested.
7. Participate in the selection process, orientation, evaluation and supervision of assigned direct support professionals as requested.
8. Promote, support, and encourage current trends and/or philosophies within the human
services field in accordance with Access Services’ philosophies.
9. Provide direct care support to individuals when assigned including day, evening and weekend hours.
10. Maintain service notes, progress notes, and other documentation as requested.

Essential Working Conditions/Physical Demands: Work in program sites and the community routinely providing direct care and support with individual(s). Stand, sit or run by self, or to provide proper care and supervision with an individual. Able to move and stay with an individual to keep them safe, both in home environment and community, both at near and far distances, even when an individual may struggle to meet identified expectations. Able to assist an individual who has fallen, regardless of weight or size. Walk, climb stairs by self, or to provide direct care ensuring individual’s safety.
