In this 180-bed sub-acute and long-term care facility, skilled professionals provide care for residents age 35 and over who have complex plans of care. This facility works closely with the Riverview Medical Center and is Medicare and Medicaid-approved.

Responsibilities

The Assistant Director of Nursing (ADON) for Long Term Care reports to the Director of Nursing (DON). S/he works with the Director of Nursing to develop and maintain standards of nursing and medical services. S/he assists the Director of Nursing with planning, organizing, coordinating, evaluating and improving the clinical practice activities of the nursing department.

Helps ensures the nursing department's compliance with all regulatory requirements for long term and sub-acute care. Serves as Director of Nursing in his/her absence.

Displays theculture of the organization and leads by example.3. Plans for safe, effective, efficient, appropriate and therapeutic nursing care.4. In collaboration with the DON, ensures that the nursing department complies with the facility's policiesand procedures.5.

Utilizes effective communication skills when working with residents/patients, families, and teammembers. Is sensitive to the individual needs of residents/patients, families and team members.Consistently displays genuineness in communicating with others. Demonstrates ability to be animpartial listener to all concerns brought to their attention.6.

Displays industriousness, conscientiousness, and diligence in performing role. Completes walkingrounds on all units.7. Shares 24-hour accountability for meeting standards of care with nursing leaders.

Assists with trackingall resident/patient acuity levels for daily staffing patterns. Recommends to the DON the number andlevels of nursing team members needed to provide quality and safe care.8. Assists with planning, developing, organizing and implementing departmental goals and objectives.Effectively communicates management decisions to achieve understanding and acceptance.9.

Demonstrates effective communication skills both verbally and in writing. Assists with conductingnursing team member and nurse leader meetings on a regular basis. Contributes to developing asense of harmony among the nursing team.10.

Assures that documentation by nursing team members is in compliance with the facility's policies andprocedures. Participates in the review of charts for proper documentation.



  1. Promotes effective liaison relationships between nursing team and other health team members.Collaborates with members of other departments to coordinate interdepartmental/interdisciplinaryactivities within and outside the facility to improve quality and safety of resident/patient care.12. Identifies and investigates concerns and issues by being detailed oriented and using strong analyticalskills.

    Develops, implements, and maintains a Quality Assurance Performance Improvement (QAPI)program for the nursing department. Analyzes Quality Indicator data and reports, identifies areas ofconcern and implements corrective action to improve resident/patient clinical processes and outcomes.13. Assists with ensuring that nursing audits are conducted, and data analyzed to improve the quality andsafety of resident/patient care.14.

    Assists in developing and implementing action plans to improve compliance with infection preventionand control standards of practice.15. Actively participates in facility committees and community programs as requested.16. Recommends staffing, programs and/or equipment needed for the budget planning process.

    Assistswith identifying reasons for budget variances.17. Conducts thorough investigation of occurrences as directed. Participates in and/or conducts RootCause Analysis (RCA) for all precursor/serious safety events.

    Assists with tracking, analyzing,reporting, and implementing plans of correction for occurrences in compliance with HMH Networkpolicies and procedures. May report results to monthly Quality Assurance Performance Improvement(QAPI) committee as directed.18. Assists with interviewing applicants for nursing positions, promotions and transfers.

    Demonstratesproficiency in Human Resource management.19. Evaluates, coaches, counsels, and disciplines nursing team members as needed to ensure the deliveryof quality and safe care. Assists with conducting investigations and participates in disciplinary actions inconsultation with the DON, Human Resources, OSHA, Department of Health and/or Ombudsman.20.

    Promotes the professional growth and development of nursing team members. Assesses and plans foreducational needs of nursing team in conjunction with the DON/Staff Development Educator andsupports nursing educational programs.21. Assures that problems and complaints involving nursing department are managed in a timely andappropriate manner.

    Follows up on all grievances and provides appropriate service recovery asdirected.22. Provides appropriate team member recognition for exceeding standards, incorporating the HMHrecognition program process wherever appropriate.23. Performs all other duties as assigned.

Qualifications

Knowledge, Education and Skills Required:1. Completion of an accredited Nursing LPN and or RN program2.

Current with all Continuing Nursing Education (CNE) requirements3. Perform clinical care professionally4. Excellent interpersonal and leadership skills5.

Demonstrated resident/patient advocate and role model to other team members6.

Demonstrated history of critical thinking and problem-solving skills7. Ability to motivate a workforce8.

Demonstrated ability to deliver resident/patient care with minimal disruptions and obstacles9. Ability to effectively work on an interprofessional team10. Proficient with extensive related computer applications

Knowledge, Education and Skills Preferred:1. Baccalaureate and master's degree in nursing preferred2.

Minimum of 2-3 years nursing experience in administrative and/or supervisory capacity3. Long term or subacute care facility experience4. Prior work experience with geriatric and rehabilitative populations5. Knowledge of latest healthcare technology and trends

Required Certification/Licensing:1. NJ LPN and or RN License2. Basic Life Support (BLS)3. Intravenous Certification4. National nursing certification (CDONA/LTC, geriatrics, rehabilitation, etc.)

Performs the duties and responsibilities of a Physician Assistant according to customary and generally accepted medical standards. This is a fulltime Physician Assitant position for the surgery department at Riverview University Medical Center in Red Bank. PA- 40 hours 4 10's with every other weekend. No Holidays, no call. Flexible schedule Responsibilities 1. Obtains a details and accurate history, and identifies problems and presents information to the supervising physician. 2. Charts via written, dictation, or electronic means: admission notes, history and physical examinations, consultations, daily patient encounter notes, urgent evaluation notes, minor procedure notes, and brief post-operative notes as it pertains to patients in the inpatient or outpatient setting; present this information to the supervising physician. 3. Prescribes medications in accordance with federal and state laws and regulations under supervision of licensed physician if approved by the New Jersey Board of Medical Examiners. Under the guidance and direction of supervising physician and in accordance with the New Jersey Board of Medical Examiners, Prescribe controlled dangerous substances only with current state CDS license and DEA number; the physician¿s assistant signs his/her own name and license number, as well as the supervising physician. 4. Assists a supervising surgeon in the operating room when a qualified assistant physician is not required, and a second assistant is deemed necessary by the supervising surgeon. 5. Assists supervising physician in a procedure room when a qualified physician is present and it is deemed that a qualified assistant physician is not needed. 6. Writes post-operative/post procedure note(s) and order(s) for supervising physician. 7. Starts IVs, inserts foley catheters and nasogastric tubes, irrigates foley catheters, orders transfusions of blood and products, venipuncture, and debriedments. 9. Exhibit organizational, communication and leadership skills, initiative, accountability, responsibility, appropriate judgment, and decision making. 10. Assist supervising physician with office hours and functions as an advocate for patients by managing prescription prior authorization, serving as a clinical expert to obtain surgical prior authorization requiring validation of medical necessity, and resolve issues associated with patient messages that require clinical support. 11. Participates in research trails collecting, aggregating and entering data. 12. Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.). This process includes communicating with the patient, parent, and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.). 13. Function as a resource to health care personnel and participate in their educational programs. 14. Greets patients and visitors in a prompt, courteous and helpful manner. Smiles, making patients/visitors feel welcome and important; makes direct eye contact; responds promptly to patients' needs. 15.
