Assistant Director Of Nursing (Adon)

Ageless Living Home Health is seeking a dynamic ADON/RN Case Manager to join our growing home health agency. We are looking for a career minded individual who will represent our mission statement and meet the standards and expectations we uphold.

This is an office based position located in Colorado Springs, CO. The Position: We are looking for a full-time Assistant Director of Nursing/ RN Case Manager with experience in Private Duty Home Health, RN Case Management, or related position. Job duties include but are not limited to:

Effectively oversees the management of patient care and the associated clinical staff (RN, LPN, CNA, etc.) in the field Evaluates appropriateness of each patient’s plan of care and revise on an ongoing basis with communication to all other disciplines Perform comprehensive assessments of physical, emotional, psychosocial and environmental needs of all assigned patients Effectively works alongside the DON/Chief Clinical Officer to ensure the continuity, quality and safety of services delivered in compliance with state and federal regulations. Position Requirements: Must have extensive knowledge of state and federal guidelines and familiar with the home health industry Must possess the ability to lead and motivate agency staff Must work well under pressure, be detail oriented, have excellent communication skills and be a team player with proven decision making skills Must possess strong clinical skills.
