• The Brethren Home Community is searching for an experienced Licensed Practical Nurse to fulfill the role of the Assistant Director of Personal Care. This unique position enables an LPN with the opportunity to combine their experience as a skilled clinician, educator, and supervisor in a vibrant and growing community.

This opening is for a full-time salaried position working Monday through Friday 8:00 a.m.

• 4:30 p.m. Occasional evening, night and weekend hours may be required.

Mission Point Personal Care at Cross Keys Village, more commonly referred to as PC Mission Point is designed for individuals who wish to remain independent when staying at home has become impractical or unsafe. Some of our residents might have limited mobility whereas others might be experiencing various degrees of memory loss. Brookside Memory Care Residence at Cross Keys Village, is a specialized personal care environment for individuals with a diagnosis of neurocognitive disorder or dementia, designed to provide an optimal environment during the intermediate stages of their disease. CKV team members work together to provide Brookside residents with guidance, cueing, the right level of stimulation and interaction with others who understand the progression of their condition.

Job Description:

As the LPN Assistant Director, your scope of responsibilities will include but is not limited to:

• Assisting the RN Director in the clinical oversight of Personal Care at Cross Keys Village, including Mission Point, Brookside and Lifespring (Adult Day Program

• Coming Soon). Providing hands-on care, as needed, in collaboration with the team of LPNs, Personal Care Attendants and Resident Assistant under the consultative direction of the RN Director, and in accordance with CKV policies and procedures as well as state and federal regulations.

• Carefully Reviewing Resident conditions each day. Conducting regular rounds on all neighborhoods. Demonstrating strong clinical competency. Evaluating the status of critically ill Residents, new admissions, and transferred Residents. Facilitating timely and appropriate communication regarding the Resident's condition with physicians/medical providers, the Resident and family members, and appropriate members of the caregiving team.

• Working collaboratively with the Brookside Memory Care Administrator on the assessment and support plan process to ensure initial and updated assessments and support plans are current, person-centered and address the care needs of the resident. Actively participating in the support plan meetings and family conferences.

• Participating fully in Department of Human Services surveys, submits reportable events and assists with developing and implementing plans of correction. Participating in 24-hour on-call responsibility's.

• Assisting with educating, directing, and evaluating the Personal Care clinical services teams. Assisting with conducting performance reviews. Providing coaching and instruction as needed. Fostering accountability and addressing concerns in a timely manner. Interviews, evaluates, and disciplines staff in conjunction with Human Resources policies when necessary.

• Conducting medication administration education to Personal Care Attendants and Resident Assistants. Identifying areas of opportunity for staff education and assisting with arranging for such education in a timely manner.

• Works with the RN Director and the Brookside Memory Care Administrator to promote and support the organizations' goals and objectives and ensures that the information is shared in a positive and effective manner with the entire clinical services department.

• Ensuring compliance with all licensing regulations and assisting with compiling data for the Quality Assurance and Performance Improvement (QAPI) and Compliance Committees.

• Covering for the RN Director and Brookside Memory Care Administrator during times of absence.

Education and Experience:

• Must be licensed as a Licensed Practical Nurse in the State of Pennsylvania.

• Minimum of three to five years of supervisory management experience in a healthcare environment.

• Memory care experience is strongly preferred.
