Responsibilities

The Assistant Clinical Supervisor will be responsible for providing patient centered, strength-based medication adherence services using interventions to assess, identify, and address SDOH for people living with HIV, through identifying and exploring barriers preventing patients from engaging in medical care and adhering to HIV medication in order to minimize barriers to care and adherence to Antiretroviral Therapy (ART). The position will utilize a team approach to provide support to the patient on implementing and sustaining the clinical treatment plan, use evidence-based intervention (motivational interviewing and social cognitive theory-based interventions) to elicit information from patients to complete a comprehensive assessment/reassessment including a SDOH assessment, to better understand health equity issues faced by the patient. Develop, in collaboration with the patient, a care plan that addresses medical, behavioral, and psychosocial functioning factors.

Through a collaborative effort with the patient's care team, the Assistant Clinical Supervisor will coordinate and advocate for services that will promote health equities and overall optimum health outcomes.

Qualifications

EDUCATION

Master's degree in Social Work (MSW), Counseling or Mental Health Counseling. NYS Department of Education License Required-will receive clinical supervision from a LCSW.

This is the process will be required for MSW candidates working towards a LMSW:- Apply to sit for the LMSW exam (this must be done before a limited permit can be requested)

  • Apply for the LMSW Limited Permit - if accepted this will cover her for 12 months.- Schedule LMSW exam date - At a minimum, will be able to take the exam 2-3 additional times during the 12-month period, if needed.

QUALIFICATIONS

  • Bilingual Spanish/English required.• Valid driver's license required.• Access to personal car for work-related travel required.• Computer operations• Possesses well-developed and effective interpersonal skills and can communicate effectively verbally and in writing.• Demonstrates confidence in actions and exercises good judgment.• Displays leadership ability and the willingness to assume authority and accountability for the direction and supervision of others.• Possesses the ability to plan, organize, develop, and implement goals, objectives, policies, and procedures necessary for quality care.• Embraces and adapts to change.• Demonstrates ability to recognize problems, approach them in an objective manner, reach appropriate solutions, implement them, and evaluate for effectiveness.• CASAC or Qualified Health Professional preferred.• At least two years' experience in a health care setting, HIV/AIDS and/or alcohol/substance use program.
