Liberty Healthcare conducts assessments and needs-based eligibility determinations for North Carolinians who are applying for Medicaid-funded services. These assessments help individuals receive appropriate levels of home and community-based services.

Due to anticipated organizational growth and an expansion of services, Liberty is now hiring for a newly-created full-time "Assessor" position.

Your typical day as an Assessor with will involve:

  • Driving throughout the region and meeting with clients in their homes

  • Administering a standardized assessment to each client

  • Assessing each client's overall health status and other pertinent health factors

  • Using a laptop to complete a comprehensive report which will be used to determine the client's eligibility for various Medicaid-funded services

  • Providing consultation to clients and helping them make informed choices about care and services

  • Collaborating with various stakeholders to help individuals live as independently as possible in their home / community

You're a good fit for this position if you possess these attributes:

  • Bachelor's degree in Social Work, Psychology or similar health and human services field

  • At least two years of experience providing case management or direct care services to people in a long-term care setting, nursing home, medical or mental health facility, general health care setting, individual or group home, or other community health setting

  • Person-centered thinking, planning and interviewing skills

  • Ability to effectively serve people of all ages who have a wide array of abilities and disabilities

  • Ability to use a laptop effectively

  • Willing to use your vehicle to travel throughout the region each day

Familiarity with some of the following areas is a plus: CMS / NC Medicaid, Home and Community-Based Services (HCBS) waivers, Personal Care Services (PCS), Community Alternatives Program for Disabled Adults (CAP DA), and Community Alternatives Program for Children (CAP C).

VIEW JOBS 9/13/2019 12:00:00 AM 2019-12-12T00:00 What You'll Be Doing Low Income Energy Assistance Program (LIEAP), Emergency Assistance under Temporary Assistance for Needy Families (TANF-EA), Energy Neighbor, Wake Electric Round Up and Warmth for Wake. The primary focus is the Crisis Intervention Program (CIP). CIP is a program that provides assistance to low-income families who are experiencing or in danger of experiencing a heating- or cooling-related emergency. A household is in a crisis if it is currently experiencing or is in danger of experiencing a life-threatening or health-related emergency, and sufficient, timely and appropriate assistance is not available from any other source. This work unit also administers additional funding sources and programs to assist individuals with their utilities. Wake County Human Services has up to 20 temporary staff employed seasonally in the Energy Program. Selected candidates will be referred to a temporary agency to finalize the employment process. Assignments are expected to begin in mid-October with some of the assignments continuing until mid-April. The positions will focus on the Low-Income Energy Assistance Program (LIEAP) but may occasionally assist with the Crisis Assistance Program (CIP). The Temporary HS Technicians for Energy Assistance will: * Interview and assess individuals and families who are applying for the LIEAP – either in person or by submitting completed application (mail, email, fax, drop-off). * Evaluate each household thoroughly on a case by case basis. * Conduct family-centered assessment, determine need/crisis, determine appropriate resource and amount, and complete appropriate application/processing requirements. * Maintain a complete and up-to-date knowledge of other community resources or programs and make appropriate referrals. * Maintain professional relationship with several WCHS vendors providing heating/cooling services for our customers * Work successfully in a high-volume and fast-paced environment Posting End Date; 7:00 pm 10/8/2019 About Our Team Wake County Human Services (WCHS) is a consolidated agency that provides an array of public health and social services that include public health and healthcare clinics, child welfare and economic services, transportation, and other related services.
