Liberty Healthcare conducts assessments and needs-based eligibility determinations for North Carolinians who are applying for Medicaid-funded services. These assessments help individuals receive appropriate levels of home and community-based services.
Due to anticipated organizational growth and an expansion of services, Liberty is now hiring for a newly-created full-time "Assessor" position.
Your typical day as an Assessor with will involve:
Driving throughout the region and meeting with clients in their homes
Administering a standardized assessment to each client
Assessing each client's overall health status and other pertinent health factors
Using a laptop to complete a comprehensive report which will be used to determine the client's eligibility for various Medicaid-funded services
Providing consultation to clients and helping them make informed choices about care and services
Collaborating with various stakeholders to help individuals live as independently as possible in their home / community

You're a good fit for this position if you possess these attributes:
Bachelor's degree in Social Work, Psychology or similar health and human services field
At least two years of experience providing case management or direct care services to people in a long-term care setting, nursing home, medical or mental health facility, general health care setting, individual or group home, or other community health setting
Person-centered thinking, planning and interviewing skills
Ability to effectively serve people of all ages who have a wide array of abilities and disabilities
Ability to use a laptop effectively
Willing to use your vehicle to travel throughout the region each day
Familiarity with some of the following areas is a plus: CMS / NC Medicaid, Home and Community-Based Services (HCBS) waivers, Personal Care Services (PCS), Community Alternatives Program for Disabled Adults (CAP DA), and Community Alternatives Program for Children (CAP C).
