Institute on Aging ("IOA") works to enhance the quality of life for adults as they age by enabling them to maintain their health, well-being, independence and participation in the community. We serve a diverse population of older adults and disabled adults by providing innovative, community-based programs that enable our clients to live at home for as long as possible. We serve as an essential partner in the continuum of care by providing health services, social and emotional support, and education and advocacy.

The AWD- Home Delivered Meals Department is seeking an Assessment Coordinator to join its team in San Francisco. The Assessment Coordinator is the primary voice in determining eligibility for home delivered meals which serve adults with disabilities in the San Francisco area aged 18-59 years. The Assessment Coordinator is part of a fast-paced program which relies on frequent field visits throughout all of San Francisco's neighborhoods, and close teamwork with IOA program staff, as well as with participating Home Delivered Meal Agencies, and the Department of Aging and Adult Services. The Assessment Coordinator conducts intake, screening, assessment/reassessment and referrals for prospective and established consumers of the AWD-HDM program.


  • Review of home delivered meal referrals made through the Department of Aging and Adult Services' (DAAS) SFGetCare database; outreaches and evaluates potential consumers for changes in condition since referral as well as risk of malnourishment or institutionalization due to insufficient food intake.

  • Conducts comprehensive in-home assessments and reassessments during which potential consumers' eligibility and appropriateness for home-delivered meals is determined. Identifies most appropriate method of service at that time for each individual based on available programs and resources in client's area.

  • Assists in linkage of potential consumers to appropriate Home-Delivered Meal program based on consumer's location, meal type and frequency needed.

  • Conducts annual in-home assessments evaluating current clients for continuing program eligibility, and monitors for any changes in functional levels, health condition and support system.

  • Provides information and referral service regarding community resources available to adults with disabilities in the San Francisco area.

  • Completes routine documentation, billing eligibility documentation and maintains client records in electronic client record (CAGetCare)

  • Enters information gathered during comprehensive in-home assessment into electronic client record (CAGetCare) highlighting risk areas and specific needs of each individual assessed.

  • Reviews CAGetCare record, and internal database to identifies individuals due for annual in-home comprehensive re-assessment and initiates outreach to client to arrange the home visit.

  • Initiates outreach to clients on waitlist not yet connected to a meal delivery provider, monitors individuals for changes in condition, presence of new or heightened risk factors, and continuing interest in program.

  • Initiates and maintains collaborative relationships with agencies and other organizations serving AWD populations through outreach activities, such as mailings, e-mailings and phone calls to program managers.

  • Participates in quality assurance according to San Francisco Department of Aging and Adult Services Office On Aging policy as well as IOA policy and procedures.

  • Follows up on delivery interruptions/clients with patterns of missed delivery as reported by meal programs and communicates resolution/developed plan to meal program.

  • Identifies environmental risks, health and safety risks (to self or others) as well as situational/interpersonal risks that client is facing, and initiates consultation and/or reporting with Adult Protective Services, SF Mobile Crisis, and/or 911 as appropriate. Follows up as appropriate on client safety issues and cooperates in identifying and establishing plan to as they relate to client safety/continued connection to meal services.

  • Works through disenrollment process for consumers who are not cooperating with, or wish to discontinue participation in the program, and

  • Attends meetings with meal provision agencies for status updates, troubleshooting, and targeted follow up with delivery sites.

  • Assists in orientation of new hires through peer shadowing.

  • Follows processes and protocols outlined in the program's Policy and Procedures, as well as agency's Emergency and Disaster plan

  • All other reasonably related tasks as assigned.


  • One-year experience working within a community-based program with disabled adults or a similar target population.

  • Exceptional communication and presentation skills relating to seniors and/or adults with disabilities, their support systems, and teams of care professionals.

  • Demonstrated assessment skills and experience in the community-based care system.

  • Ability to prioritize workflow and manage multiple and varied work

  • Highly organized and detail oriented with good problem-solving skills

  • Computer literate


  • BA/BS in Social Work, Psychology, Sociology, Gerontology, or Nutrition-related degree
