Role:

The After Hours Hospice Nurse Practitioner (APRN) is responsible to provide night, weekend and/or holiday telephone coverage during assigned shifts for hospice patients among the hospice affiliates. Provides high quality care that is consistent with the hospice plan of care and patient/family goals of care.

Qualifications:

  • Master's degree in nursing

  • APRN certification recognized by CMS; certification in their discipline by an organization recognized by CMS and the State of Florida

  • Licensed to work as APRN in the State of Florida

  • Active BLS for healthcare professionals from the American Heart Association or Red Cross. Employees hired prior to 12/31/2022 must obtain certificate prior to 10/1/2023

  • Current DEA number prior to beginning employment

  • Current Medicare and Medicaid Numbers

  • Eligible for malpractice coverage under the Organization's policy terms

  • Knowledge and experience in palliative medical practice and symptom management

  • Strong interpersonal and communication skills

  • Basic computer skills including use of email and word processing (Microsoft Outlook and Word or equivalent programs)

  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families

  • Ability to prioritize, triage, and manage clinical calls and information provided

Job Responsibilities:

  • Works collaboratively with the collaborating Physician and the interdisciplinary group (IDG), fostering collaborative relationships in the provision of high quality, cost-effective palliative care to hospice patients.

  • Maintains high quality, timely clinical documentation.

  • Prescribes pharmacologic agents to ensure that patient medical needs are met.

  • Responds to request of change in patient condition, resulting in level of care change that requires transfer in or out of general inpatient (GIP) setting; hospice house or contract hospital, or need for continuous home care.

  • Provides telephone medical coverage for all hospice patients, including those whose non-hospice attending Physician desires consultative services or cannot be reached.

  • Communicates clinical information to appropriate staff at the completion of shift.

  • Leads specific projects as directed by the Staffing Services Manager, Medical Director, or Corporate Medical Services.

  • Participates in medical reviews, as needed, and directed by the Medical Director or other Physician.

  • Acts as a liaison and consultant to the attending Physician and other members of the interdisciplinary group.

  • Performs other duties as assigned.
