Are you passionate about primary care and enjoy serving a diverse patient population? Then consider this full-time Advanced Practice Provider opportunity with our team at Lakeland Primary Care, Hartford.

Will consider Physician Assistants (PA) or Nurse Practitioners (NP). Prefer AGNP-trained nurse practitioners. Strongly prefer providers with experience managing patients with chronic and psychiatric conditions. Team includes two physicians. This role will share a patient panel with our internal medicine physician. Epic EHR; Fluency dictation available.

Fluency or ability to conversationally speak Spanish a plus. Practice serves a diverse patient population. Monday

  • Friday schedule, 8am

  • 5pm. On call for abnormal test results and provider patient panel only. Ability to work with NP or PA students if desired.
