TITLE: Advanced Practice Provider (Nurse Practitioner and/or Physician's Assistant) Immediate Care

REPORTS TO POSITION: SCMG Chief Medical Officer (or designee)

DEPARTMENTAL SUMMARY: The Immediate Care Centers provide urgent care services to all presenting patients with Emergency Severity Index (ESI) of 4 and 5 with some ESI 3's as may be deemed appropriate. Patients who present requiring a higher level of care will be assisted in arriving at the appropriate higher level of service. The ICCs interact collaboratively with primary care, emergency care and specialty providers from within St. Charles as well as other non- associated entities.

POSITION OVERVIEW: The St. Charles Immediate Care Centers are staffed by Physicians and Advanced Practice Providers (APPs) who are skilled in providing urgent and acute care. Requirements include the ability to work in a collaborative environment with Physicians to provide rapid and efficient care for minor emergencies and illnesses. Other duties can include performing basic physical exams such as sports physical and occupational exams. The ability to work efficiently and refer appropriately is crucial. The ability to stabilize patients who present needing a higher level of service while awaiting transport is required. The ICC's function as a unified department and shifts will be rotated among the locations. Lab and X-ray are integrated into the department. APPs should have emergency department, urgent care, or primary care with substantial acute care experience.

Work schedule: APPs to follow a shift-based model with a base reconciliation of .75 FTE scheduled at 141 shifts per calendar year. Shifts to be 10 hours in length.

Call expectation: APPs are not expected to have call expectations in the Immediate Care.

Work location: APPs primary work locations include Bend South Immediate Care and La Pine Immediate Care.

ESSENTIAL FUNCTIONS AND DUTIES: (May perform additional duties of similar complexity within SCHS as required or assigned)

Clinical Expectations:

  • Practice and role model excellence in the Immediate Care domains of diagnosis, aligning treatment goals to patient values, definitive treatment, supportive care treatment, follow-up care, promote good health and prevent illness.

  • Practice medicine within the scope of license, training and credentialing. Cooperate with other SCHS providers and caregivers to insure optimal patient care.

  • Assess, plan, implement and evaluate individual patient care while demonstrating expert knowledge of evidence-based, Immediate Care.

  • Document all patient care in the proper form and in a timely manner in compliance with hospital policies and regulatory agencies.

  • Delivers first-rate customer service and/or patient care in a manner that promotes goodwill, and is timely, efficient and accurate, and courteous.

  • Calculates dosages and orders medications accurately in accordance with SCHS policies and procedures for best practices for of medication administration.

  • Diagnosing and managing minor illness and injury including children; stabilization and appropriate transfer of patients requiring higher level of care; perform emergency patient codes per ACLS and PALS guidelines.

  • Repair of appropriate lacerations; incision and drainage of abscesses and treatment of skin infections; diagnosis and initial management of fractures and dislocations; foreign body removal; ingrown toenail removal; diagnosis and treatment of eye infections and foreign bodies (including use of slit lamp).

  • Initial interpretation of x-ray; EKG interpretation; review of returning lab and x-ray reports and assigning appropriate dispositions and follow-up.

  • Willingness to provide short term medication refills (non-narcotic or scheduled) for patients in provider transition; review of appropriate prescription monitoring data bases and conforming to our policy of limited narcotic prescribing.

  • Provide a therapeutic presence in service to others by purposefully responding to the needs of patients/customers in a caring way. This includes introducing oneself and stating your purpose, asking the patient their preferred name, spending time with the patient to determine their care goals, active listening, and using skillful communications (appropriate touch, eye contact, etc.).

  • May perform additional duties of similar complexity within the organization, as required or assigned.

Administrative Expectations:

  • Document all patient care within the Electronic Medical Record (EMR) and complete all necessary tasks in compliance with health system policies and regulatory agencies to ensure timely and accurate billing.

  • Be familiar with the SCHS organization structure.

  • Participate in SCMG provider meetings and assist in policy formation for SCMG and providers practice.

  • Keep medical knowledge and skills current through regular CME activity. Must attest to having adequate (CME) to meet maintenance of board certification and compliance with MSO rules and continuing privileges without restriction.

  • Keep all licenses and appropriate certifications current and unrestricted to maintain privileges at a St. Charles Health System hospital sufficient to permit performance of job duties.

  • Complies with all applicable laws, regulations, policies and procedures, supporting the health system's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all health system's investigations and proceedings.

  • Acts as a leader and supports departmental goals and the vision, mission and values of the St. Charles Health System in all respects.

  • Supports Value Improvement Practice (VIP- Lean) principles of continuous improvement with energy and enthusiasm, functioning as a champion of change; Contribute as necessary to assist in policy formation for the organization.

  • Maintain compliance of hospital's Medical Staff Bylaws, Rules and Regulations, as required by SCHS Board of Directors.

EDUCATION

Required:

  • Graduation from an accredited Physician Assistant or Nurse Practitioner program.

  • Physician Assistant requires NCCPA certification. Nurse Practitioner requires AANC certification. If Physician Assistant, ability to establish and maintain a Practice Agreement with a Supervising Physician.

  • Complete annual fire/safety, etc. education in-services as required.

  • Attend mandatory educational offerings as described in SCHS policies/procedures and/or as required by other regulatory bodies.

  • Complete annual CME as required to maintain license and appropriate Board certification.

  • Complete by assigned due date Computer Based Learning modules.

LICENSURE/CERTIFICATION/REGISTRATION

Required:

  • Current license as PA/FNP.

  • Unrestricted Federal DEA number.

  • Unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs.

  • Valid driver's license and the ability to meet SCHS driving requirements.

  • Current BLS certification required.

  • Current ACLS and PALS certification required.

  • Additional certifications may be required based on privileges granted to provider by Medical Staff Services at SCHS Inpatient locations.

Preferred:

  • Oregon license as PA/FNP.

  • Federal DEA number registered in Oregon.

EXPERIENCE

Required:

  • Experience using EMR (Electronic Medical Records) software.

  • Advanced written and verbal communication skills to effectively interact with a diverse population and professionally represent St. Charles Health System.

  • Experience working in a team environment and proven ability to collaborate, manage facilitation and consensus building among health care professionals and agencies to achieve expected results.

Preferred:

  • 2 years of medical practice in emergency medicine, urgent care, or primary care with substantial acute care component

  • Epic EMR software experience.

PERSONAL PROTECTIVE EQUIPMENT

  • Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

  • Intermediate to advanced proficiency with Electronic Medical Record systems.

  • Intermediate in Microsoft applications (Word, Excel).
