Join our team at GGH as an AWV Nurse Practitioner and play a crucial role in providing comprehensive care to senior Medicare patients. In this position, you will conduct annual wellness exams, create personalized prevention plans, and optimize the management of patients' medical conditions. Key Responsibilities:

• Conduct annual wellness exams for senior Medicare patients, either in their place of residence or in a designated clinic

• Obtain and record patients' medical history, symptoms, and medication reconciliation.

• Perform various tests, screenings, and accurate coding.

• Collaborate with the Primary Care team to ensure holistic patient care.

• Maintain timely completion of charts following patient visits using EMR and/or portals.

Requirements:

• Current California Nurse Practitioner license and certification from either the ANCC/AANP.

• 2+ years of clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care. Home care experience is a plus.

• Working knowledge of chronic care management and medication options.

• Current DEA licensure/Prescriptive Authority and active CPR/BLS Certification.

• Bilingual proficiency in English and Spanish preferred.

• Valid California Driver's License.
