Primary responsibilities of Annual Wellness Nurse
identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and execution of guided processes to complete the AWV
review of all chronic conditions with supporting documentation, ensuring the physician is coding each diagnosis to the highest level of specificity, ensuring patient adherence to the medical plan of care, educating as needed on evidence-based medicine guidelines, including all preventive and disease specific screenings
Assists patient with the development of self-management goals or appropriately refers to Chronic Care management
Verifies eligibility, HCC codes needing addressed, closes care gap opportunities and develops a 5-10 year preventive plan of care using the USPSTF recommendations
Contacts and schedules patients as needed
May also assist clinic staff with completion of other clinical tasks as needed
Knowledgeable of community resources and assists patients in connecting with those resources
Serves as patient advocate and assists in identification and improvement of service delivery
This position requires expertise in the nursing process, using critical thinking skills to plan and coordinate care. The AWN is responsible and accountable for direct and indirect patient care for designated patient populations.
Although, primarily performed by a LPN, the role of an Annual Wellness Nurse can be accomplished as an RN with understanding that salary will be established at an LPN level.
Primary responsibilities of Performance Improvement Specialist
Conducts medical record reviews as identified through the medical staff programs
Assists in the assessment of anticipated input of external regulation pertaining to Quality Assessment/Performance Improvement
