The Ambulatory Social Worker (LMSW) is as a member of the Ambulatory Care Management team and is accountable for improving the health outcomes of the populations served. The Ambulatory Social Worker works as part of a multi-disciplinary team under the direction of the Ambulatory Care Management leadership. The Ambulatory Social Worker is responsible for managing the coordination of behavioral health services and addressing the social determinants of the covered population. The role will participate in all components of care management including, but not limited to, management of care transitions; provide individual-centered education; evaluate for social determinants or barriers that impact care management; adherence to quality metrics; facilitate communication among providers; and advocate for the individual, family, and support system.

Responsibilities

    1. Assess the biopsychosocial status of identified individuals, including an evaluation of social functioning particularly as it relates to medical problems.

  • 1.1 Interview individuals, families, and caregivers for personal, social, and environmental history.

    1. Demonstrates an ability to serve as a collaborative member of a multidisciplinary healthcare team.

  • 2.1 Assist in interdisciplinary team meetings to meet the psycho-social needs of individuals to enable them to utilize medical, mental health, and community services.

    1. Demonstrates knowledge of available patient support, including technology and community resources.

  • 3.1 Maintain familiarity with community resources and agencies for assigned regions and update resources as necessary.

    1. Demonstrates an understanding of clinical standards, quality performance goals, and expected outcomes.

  • 4.1 Demonstrates application of evidence-based practice and clinical practice guidelines to care plans and patient interventions.

    1. Performs other duties as required or requested.

Qualifications

EDUCATION

Master's degree from an accredited school of social work.

EXPERIENCE

Minimum of two (2) years of experience with direct patient care in an acute, community, or post-acute provider or healthcare setting.

LICENSURE

Licensed Master Social Worker (LMSW) from the State of Connecticut, Licensed Clinical Social Worker (LCSW) from the State of Rhode Island, or those holding licenses from other states, will need an 'endorsement' and meet the State of Connecticut or Rhode Island license requirements.

SPECIAL SKILLS

Motivational interviewing skills necessary. Excellent verbal and written communication skills. Possesses excellent organizational skills and ability to handle multiple priorities. Ability to work in an independent role with minimal supervision. Ability to work collaboratively with health care professionals at all levels to achieve established goals and improve quality outcomes. Working knowledge and ability to apply professional standards of practice in a work environment. Working knowledge of computers and basic software applications used in job functions, such as word processing, databases, spreadsheets, and others as needed.

PHYSICAL DEMAND

The role is primarily a remote work position with the ability and expectation to travel to onsite practice locations from time to time as requested by management. Ability to attend initial onsite orientation and onboarding program as well as monthly onsite training obligations. Adheres to all organizational remote worksite standards.
