The RN Care Coordinator Triage (RNCCT) supports the improvement of patient care by increasing engagement between the primary care team and the patient while helping to empower patients and their families with the information they need to better manage their health. Within the RN scope of practice, the RNCCT is expected to consistently demonstrate competent, focused, clinical nursing assessments and history taking on all new, established and acute care patients in both triage and clinical care settings.

Patient Population Served:
The RN Care Coordinator Triage (RNCCT) is expected to demonstrate the knowledge and skills necessary to provide care based on the physical ability, intellectual development, psychosocial, education, and safety needs of the child, adolescent, adult, and geriatric patient.
Essential Position Functions:
The RN Care Coordinator Triage (RNCCT) is responsible for triaging patients on the phone and/or those who walk into the practice, as well as working as part of the medical team partnering with patients and families with chronic disease self-management. Communicates and collaborates with other departments, physicians, patients and families to encourage and foster actions which meet patient needs and achieve outcome goals.
Current Licensure as a Registered Nurse in the State of New Hampshire or other Compact State. Bachelor's Degree preferred.
3-5 years recent clinical experience as RN, prior triage and/or discharge planning experience preferred.
Case Management Certification desired.
Demonstrate proficiency in all required procedures, competencies, and clinical skills.
Demonstrates ability to work well under pressure, handle multiple priorities, and exercise good judgement in a clinical setting.
Ability to communicate effectively, both verbally and in writing to a diverse audience.
Skilled with the Microsoft Office Suite (Word, Excel and PowerPoint) and EMR systems, Meditech preferred.
