SCHEDULE: Monday through Friday, Days


  • Five (5) years of acute hospital care experience

  • Excellent oral and written communication and interpersonal skills

  • Strong problem solving, decision making and critical thinking skills

  • Ability to recognize and manage emergencies

  • Strong knowledge of specific chronic care regimen

  • Time management and organizational skills

  • Must be self-directed and able to collaborate


  • Patient Care. Serve as the patient's first line of communication throughout the entire treatment program; coordinate multiple and complex technologies as part of the diagnosis and treatment; coordinate appointments and procedures; perform direct patient care, nursing documentation, symptom management, patient and family education and counseling

  • Leadership. Form a collaborative practice among the care management team (i.e. surgeons, nurses, social workers, etc.); provide community outreach and education regarding the program; respond to calls concerning the program

  • Collaboration. Work closely with the physician during patient visits; become aware of the treatment plan, expected outcomes and possible treatment complications

VIEW JOBS 11/27/2018 12:00:00 AM 2019-02-25T00:00 City Rochester State Minnesota Telecommute No Department Pediatrics Position description The role of the Aerodigestive and NICU follow up clinic RN Care Coordinator is to focus on the complex care coordination for patients and families across the continuum of care. The RN Care Coordinator (RNCC) serves as the central point of contact for patients/families and the multidisciplinary team. The RNCC will see patients in clinic and complete an assessment, review the patient care plan and goals, provide patient education and review any outside medical records. The RNCC communicates and updates the care team on significant findings and/or family concerns/goals of care. The nurse arranges and plans for clinic appointments and assures the appropriate lab work, x-rays, procedures and consultations are scheduled and coordinated for follow up. The RNCC implements treatment plans, triages phone calls, and facilitates communication across providers, settings, and caregivers. The RN Care Coordinator provides emotional support and educational needs of patients and families. They also provide needed resources for transitional care to home, school, and the primary care physician. RNCC serves as a liaison with other Mayo sub-specialty providers, as well as local health facilities and primary providers for maintaining continuity of care. It is essential that the nurse coordinator enjoy working with patients and families in complex medical and social situations and possess excellent communication and critical thinking skills. (012560) Qualifications Graduate of a baccalaureate Accreditation Commission for Education in Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program with three to five years of experience. Additional qualifications BSN required. A minimum of 2 years pediatric or neonatal nursing experience preferred. Knowledge of pediatric population characteristics and desired outcomes. Candidates must be able to work independently alongside a team of physicians within the Peds specialty. Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills. Internal applicants should attach their three most recent performance appraisals. License or certification Must obtain prior to start date: Current RN license by applicable state requirements. Arizona and Florida: Maintains Basic Life Support (BLS) competency. Midwest: Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered. We will only accept certificates of course completion from the following providers: * American Heart Association (Title: BLS for Healthcare Providers or BLS Provider) * Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers) * Military Training Network Additional specialty certification/training as required by the work area.
