Staff position on the Ambulatory Clinical Quality team. This team member is responsible for understanding learning and understanding the quality requirements of the ambulatory quality programs including, but not limited to, the ACO, CMS physician practice programs to be developed under MIPS, MIPS/APM, and the CHS Ambulatory Performance Improvement program. Data review, abstraction of data from various medical record modalities and data validation will be required as well as education and training of practice staff regarding the quality indicators and program components.
In addition, the individual will be expected to participate in corporate-wide Ambulatory performance improvement initiatives and population health focused workgroups and programs. Other responsibilities may include participating in clinical workflow build in the electronic medical record, assisting educators with the education of practitioners to facilitate compliance with quality measure reporting, assisting Data Analysts with the aggregation of performance improvement data and information, and preparing reports for leadership review.
Responsible for learning/maintaining current knowledge of quality program requirements.
Participates in corporate-wide Ambulatory performance improvement initiatives. Assists CHS network physicians and staff with understanding and reporting of performance improvement activities.
Responsible for reviewing medical records, and abstracting clinical data elements for quality measures according to published criteria.
Responsible for entering quality measure data into appropriate quality databases and interfaces to facilitate reporting.
Completes chart review, data entry and other reporting requirements by deadlines as stipulated by the regulatory agency and/or CHS Leadership.
Participates in the development of electronic workflow build to support quality measure compliance/abstraction as appropriate. Ability to navigate and understand clinical data repositories including the Epic electronic health record.
Assists with educating practitioners on program requirements, workflow, and other elements as it relates to the quality program.
Assists the Quality Data Analysts with reports that aggregate quality program data to facilitate performance improvement initiatives within the physician partners network, ACO, etc.
Completes other duties/responsibilities as assigned.
POSITION REQUIREMENTS AND QUALIFICATIONS:
NYS licensed RN, NP, or Physician Assistant
Bachelor's degree required; Master's degree preferred.
Six Sigma Green Belt or LEAN certification a plus.
Must have knowledge of practitioner care principles, procedures, and practices.
Demonstrates understanding of Hospital and Ambulatory/Practice-Based clinical environments and workflows.
Experience in working with an Electronic Health Record.
Proficiency in database, spreadsheet, and word processing software applications essential. Microsoft Office proficiency required.
Familiar with Six Sigma, PDCA and/or other performance improvement methodologies
Demonstrates the ability to effectively communicate with all levels of hospital and practice-based staff in a verbal and written manner
Possesses keen analytical skills and demonstrates the ability to be very organized.
Effectively plan projects, workflow activities, and assignments.
Minimum of 3-5 years clinical experience required.
Performance Improvement and Quality Management experience preferred.
Prior experience in clinical quality measure data abstraction preferred.
