Banner Pharmacy Services provides innovative pharmacy services and coordinated patient care across the continuum leading to improved medication outcomes. BPS clinical staff members serve as advisor to consumers, providing care, guidance and support for wellbeing through optimal medication management. In addition to that, we partner with providers to offer guidance and support positive patient outcomes through individualized medication management and proactive outreach.

You will have the opportunity to work with Ambulatory Care Clinical Pharmacists embedded within primary care and/or specialty clinics providing disease state management services while working under collaborative drug therapy management agreements. The pharmacist actively manages medication therapy, completes medication reviews and collaborates with providers to optimize medication therapy, improve medication adherence, promote wellness, prevent disease, and improve outcomes.

In addition to having the opportunity to provide comprehensive patient education to improve medication adherence and promote self-management skills and patient activation in their own care. This position will be in the Northwest Valley. Monday-Friday, working the day shift.

Job Summary

This position actively participates on the healthcare team as a pharmacotherapy expert in the area of ambulatory care to provide high quality patient care. Contributes to the optimal selection of drug therapy, design, and implementation of patient-specific medication regimens to maximize therapeutic benefit and minimize toxicity.

Provides comprehensive patient education to improve medication compliance and promote patient self-management skills. Must possess clinical acumen and skills necessary to assess and interpret data reflective of the patients clinical condition. Acts as a driving force in the identification, development, and promotion of clinical pharmacy services. Conducts educational activities, applies principles of performance improvement, and adheres to local, state, and federal laws regarding the profession of pharmacy practice.

Essential Functions

  • Performs medication therapy and disease state management, which includes critically evaluating disease states and adjusting medication therapy to optimize patient medication regimens and improve clinical outcomes. Ensures utilization of appropriate and cost-effective medication therapy. Areas of focus may include, but are not limited to, disease state management (e.g. thromboembolic disease, diabetes, cardiovascular disease, COPD), medication management (e.g. pain management), and adverse drug event prevention and management.

  • Demonstrates accurate and thorough documentation skills. Facilitates pharmacist-to-provider communication and clinical interventions.

  • Acts as a system resource and drug information expert in area of ambulatory care. Provides detailed drug information and education to health care team, pharmacy, patients, and community.

    Provides feedback and/or consultation related to technology utilization for documentation of medication therapy management, clinical pharmacy outcomes, or other identified medication related issues. Utilizes and may provide technical guidance with computer software, particularly medication management information system software. Utilizes reference tools to evaluate patient care and answer questions posed by customers. Consultant for utilization of technology related to medication therapy.

  • Contributes to the total body of knowledge of pharmacy through publication and/or educational presentations. Participates in the development and/or management of outcomes-based research.

    Applies the principles of continuous quality improvement consistent with job expectations. Participates in drug use evaluations, medication use indicators, and disease-based evaluations.

  • Acts as a driving force in the identification and development of clinical pharmacy services related to medication and disease state management at the local and system level. Participates in system based committees, which may be in the area of specialty or other related area, and effectively communicates with stakeholders.

  • Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Adheres to all clinical practices approved by Pharmacy and Therapeutics Committee/Clinical Consensus Group (e.g. collaborative practice agreements, clinical pharmacy protocols).

Minimum Qualifications

Doctor of Pharmacy degree or another advanced degree in pharmacy from an accredited College of Pharmacy.

Requires current pharmacy or eligibility for licensure in the applicable state of practice. Licensure in applicable state of practice required within 90 days of hire date.

Licensure upon hire may be required based on business needs. Board Certification in appropriate area of specialty within two years of hire.

Completion of a PGY1 accredited pharmacy residency plus 3 years of clinical pharmacy experience in the ambulatory care setting. A minimum of five years of clinical pharmacy practice experience in the ambulatory care setting or area of specialty may be considered in lieu of a PGY1 pharmacy practice residency.

Maintains good organizational, problem solving, and computer skills. Possesses good written and verbal communication skills for interfacing with all levels of staff, physicians, patients, and other contacts.

Preferred Qualifications

Completion of a PGY2 accredited pharmacy residency program in ambulatory care.

Additional related education and/or experience preferred.
