We have a new position for a Ambulatory Care Coordinator, Medical Assistant with Main Line HealthCare's Ambulatory Care Management and Coordination Services.
This position is based out of the Corporate office in Newtown Square, PA with travel to Practice locations required as needed
The Medical Assistant will be working Full Time, 40 hour/week position Monday-Friday, Primarily day shift with occasional early evening hours to meet patient needs. No holiday requirements
The Ambulatory Care Coordinator, Medical Assistant works under the supervision of the Director, Ambulatory Care Management and Coordination Services and in collaboration with other members of the health care team. This position will have a primary focus of either Care Gap Team member or Health Coach. The role assists the health care team (providers and clinical support staff) with access to care coordination activities including protocol-driven disease management services and education for Main Line HealthCare patients.
Collaborates with patients, families, primary and specialty care providers in multiple practice sites to facilitate clinical guideline adherence and coordinate services specifically related to the patient's health care needs. Follows a plan of care for assessed patient needs, education pathways, clinical guidelines and diagnostic tests to assist the patient and primary care team with individualized plans of care. Maintains documentation and collaborates with the health care team to adjust interventions to maximize clinical, quality, and fiscal outcomes. Participates in reengineering activities and quality improvement projects as required.
Responsibilities for both Care Gap Team Member and Health Coach include but are not limited to:
Participates in the design of interventions to support health management programs. Coordinates with the provider, patient and other health care team members to implement patient specific strategies to enhance chronic care and preventive care management. (
Assists with age appropriate learning principles and supports the health care team with education/clinical guidelines
Utilizes established education and clinical pathways to support comprehensive and consistent disease management education in longitudinal chronic care.
Collaborates with primary care and specialty providers as well as the health care team to enhance clinical guideline adherence and promote best practice by initiating pre-visit planning and preventive and specialty care education and scheduling services when appropriate
Assists with coordinating laboratory and diagnostic tests for all disease management programs as defined by disease management program pathways
Collects and validates data and documents as required for all programs. Works with the Population Health Team to evaluate outcomes for these programs according to Best Practice standards.
Maintains patient confidentiality and protects Protected Health Information (PHI) with all activities in Ambulatory Care Management and Coordination programs
Utilizes Care Gap outcomes to promote benefits and availability of disease/case management services with providers and other health care providers
Assists with reactive outreach and conducts follow-up interactions to high-risk populations
Assists with administrative auditing of patient encounters in support of payer programs
Identifies problems, proposes process improvement strategies and assists the Ambulatory Care Management programs in reengineering activities. This includes identification and development of key clinical initiatives to improve quality and workflow to enhance clinic efficiency and patient engagement
Attends and participates in professional development opportunities as needed to support the work Ambulatory Care Management programs
Incorporates excellent listening, verbal, written, and problem solving skills into patient interactions.
Performs other duties as required or assigned by emergency or other operational reasons for which the employees is qualified to perform
