POSITION SUMMARY: This position directs the operations of the agency and ensures that all home care support services are delivered in a safe, efficient and effective manner, in accordance with established quality standards, and applicable state and federal regulations. This position also participates in the agency's business development efforts to expand its client base and promote revenue growth.


  • Direct the overall operations of the agency and provide direction and guidance to the agency staff in scheduling and performing home care support client services.

  • Oversee the development and execution of client care plans and ensure that all services are performed in an efficient and effective manner, in accordance with established quality standards, Company policies and procedures, and applicable state and federal regulations. Intervene, as necessary, to resolve problems and ensure that all client service requirements are met.

  • Manage the agency's daily activities and administrative functions, including client/employee scheduling, personnel management, payroll processing, billing preparation and records maintenance.

  • Direct and participate in the recruitment, orientation, training and retention of agency staff. Coordinate the design and implementation of staff education programs. Ensure the agency's compliance with the Company's human resources policies and practices, as well as all related regulatory requirements.

  • Participate in the development and implementation of the agency's objectives, strategies and initiatives for census/revenue growth and business development with the execution of related sales and marketing plans.

  • Develop and maintain relationships with state and county referral agencies to promote the agency's programs and services and expand its client base. Participate in national and regional home care industry organizations to follow trends and developments and identify potential business opportunities.

  • Participate in the preparation of the agency's annual budget. Monitor the agency's performance against established revenue, expense and profitability targets. Implement appropriate corrective actions to address problem areas and/or negative trends.

  • Participate locally in the Company advocacy program to educate and favorably influence legislators and other elected officials in Congress and state government to the benefits of in-home care services, reimbursement rates and program funding.

  • Maintain a high degree of confidentiality at all times due to access to sensitive information

  • Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency.

  • Follow all Medicare, Medicaid, and HIPAA regulations and requirements.

  • Abide by all regulations, policies, procedures and standards.

  • Performs other duties as assigned.


  • Bachelor's Degree in Business, Social Services, Public Administration, or a related field of study

  • Five to seven years of experience in health care, including at least five years of supervisory/management experience

  • Experience in home health care and/or home and community based services in a large, multi-site organization is preferred

  • Demonstrated ability to drive census/revenue growth and sales/develop business

  • Computer proficiency -- MS Office -- Word, Excel, Outlook

  • P&L experience and the ability to develop and manage an agency budget

  • Strong communication, teambuilding and interpersonal skills

VIEW JOBS 1/14/2020 12:00:00 AM 2020-04-13T00:00 Position Summary: The Home Health Registered Nurse reports to the Director of Nursing and is a member of the interdisciplinary team. This role is responsible for the provision of skilled professional nursing, and assessment of patient care in order to maximize the palliation of symptoms. In addition, the Home Health RN is responsible for identifying patient and family needs and providing supportive care and provision of appropriate services in accordance with the attending physician's orders, the interdisciplinary team plan of care, and the policies and procedures of the agency. Essential Duties: * Assess, plan and intervene using the nursing process to meet the patients' health care needs, as directed * Obtain data on physical, psychological, social and spiritual factors which may impact patients' and family needs and initiate intervention by other members of the interdisciplinary team * Observe and report any signs or symptoms indicative of changes in condition of the patient and family situation. Document such reports and observations in the clinical record * Notify the Case Manager, physicians, and families of changes in the patient's condition * Maintain data collection, records, and reporting systems to ensure proper service, uniform accounting, data collection, and measurement of outcomes * Communicate potential risk management issues and report abuse or unsafe/illegal practice as required by law * Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned * Supervise and assess the delivery of care by the HHA, as needed * Share in providing 24 hour, 7 day per week coverage to patients and families, as needed * Maintain a high degree of confidentiality at all times due to access to sensitive information * Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency * Follow all Medicare, Medicaid, and HIPAA regulations and requirements * Abide by all regulations, policies, procedures and standards * Performs other duties as assigned PERFORMANCE RESPONSIBILITIES: * Maintains positive internal and external customer service relationships * Maintains open lines of communication * Plans and organizes work effectively and ensures its completion * Meets all productivity requirements * Demonstrates team behavior and promotes a team-oriented environment * Actively participates in Continuous Quality Improvement * Represents the organization professionally at all times Position Requirements & Competencies: * Graduate from an accredited registered nursing program * Licensed as a RN in the state of practice * At least one (1) year clinical practical nursing experience * Home health experience preferred * Ability to make home visits and provide nursing care in the home environment * Ability to complete necessary documentation appropriately, legibly and timely * Interpersonal, verbal, and written communication skills necessary to complete job responsibilities * Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation Working conditions: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
