POSITION SUMMARY: Responsible for the overall coordination of the referral and admissions process. Interacts closely with facility representatives, discharge planners and other agencies to facilitate the initiation of care.
Receive referrals by phone or in person for designated department
Provide explanation of the benefits to patients, families and community contacts
Assist with finding alternative services when patient is not a candidate for Ambercare
Oversee scheduling of initial nursing assessment and admission
Maintain admission packets and ensure that proper forms are available to admissions staff
Knowledgeable in all Home Health and/or Hospice processes
Assist community outreach representatives with marketing activities to promote relations with referral sources
Assist with completion of admission paperwork as needed
Adhere to work schedule and maintain good punctuality and attendance record
Provide telephone and clerical support to other staff members as time allows
Maintains accurate clinical records, keeping track of goals and progress of clients'
Abides by all regulations, policies, procedures and standards
Perform other duties as assigned
Maintains positive internal and external customer service relationships
Maintains open lines of communication
Plans and organizes work effectively and ensures its completion
Meets all productivity requirements
Demonstrates team behavior and promotes a team-oriented environment
Actively participates in Continuous Quality Improvement, educational programs, and staff meetings

POSITION REQUIREMENTS & COMPETENCIES:
Be eighteen (18) years of age or older
At least one (1) year experience in a homecare, hospice and/or healthcare related field preferred
Ability to make home visits and provide nursing care in the home environment
Ability to complete necessary documentation appropriately, legibly and timely
Ability to handle stressful and delicate situations
Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation

While performing the duties of this job, the employee is regularly required to sit, talk, hear; use hands to finger, and handle controls. The employee frequently is required to reach with hands and arms. The employee is occasionally required to stand, walk, and stoop. Specific vision abilities required by this job include close vision and the ability to adjust focus.
ENVIRONMENT: Moderate noise level similar to a typical home environment and/or typical office environment with computers, printers, and work activity.
