Since our establishment in 2009, our mission has been to provide compassionate, personalized care and support to individuals facing medical conditions, ensuring their well-being and recovery in their own homes or Assisted Living Facilities. As a Care Coordination Agency, we specialize in assisting Medi-cal recipients to enroll in the Assisted Living Waiver Program (ALW) and coordinate their smooth transition and care into an ALW-approved facility. Our goal is to offer comprehensive support and guidance throughout the process, enabling individuals to thrive in the assisted living setting. Position Overview: The Assisted Living Waiver Program RN Care Coordinator is a vital member of the healthcare team responsible for coordinating and managing the care of elderly or disabled individuals participating in the Assisted Living Waiver Program. This role combines nursing expertise, case management, and administrative duties to ensure that participants receive high-quality care and support in an assisted living setting. The RN Care Coordinator acts as an advocate, collaborator, and facilitator, ensuring seamless communication between participants, their families, healthcare providers, and relevant agencies. Key Responsibilities: 1. Care Coordination: Assess the healthcare needs and functional abilities of program participants. Develop comprehensive person-centered individual service plans (ISP) tailored to their needs and goals. Regularly review and update ISP based on changes in the participant's condition or circumstances. Coordinate with assisted living facilities, healthcare providers, and support services to ensure the proper implementation of care plans. Conduct periodic assessments to monitor the participant's progress and adjust ISP as necessary. Conduct monthly supervisory visits to participants residing in assigned assisted living facilities. 2. Participant Advocacy: Advocate on behalf of program participants to ensure their rights, preferences, and choices are respected and honored. Assist participants in understanding their healthcare options, rights, and benefits. Mediate and resolve any concerns or issues that may arise in the course of care. 3. Collaboration and Communication: Collaborate with a multidisciplinary team, including social workers, healthcare professionals, and caregivers, to provide comprehensive care. Facilitate communication and information exchange between all involved parties to maintain a holistic approach to care. Attend meetings and conferences related to the Assisted Living Waiver Program to stay updated on policy changes and best practices. 4. Documentation and Compliance: Ensure accurate and up-to-date documentation of participant information, care plans, and interventions. Adhere to program regulations, policies, and procedures to maintain compliance with state and federal guidelines. Assist with data collection and reporting requirements as mandated by the program. 5. Education and Support: Provide education and support to participants and their families regarding their conditions, medications, and self-care strategies. Collaborate with community resources to arrange educational workshops and support groups for participants and their families. 6. Quality Improvement: Participate in quality improvement initiatives to enhance the overall effectiveness and efficiency of the Assisted Living Waiver Program. Identify areas for improvement and recommend strategies for better service delivery. Qualifications: Registered Nurse (RN) with a current and valid license in the state. Experience in geriatric or long-term care nursing, case management, or care coordination is an advantage. Knowledge of Medicaid and assisted living regulations and guidelines is advantageous. Strong interpersonal and communication skills to interact effectively with participants, families, and healthcare professionals. Ability to assess participants' needs and develop appropriate care plans. Excellent organizational and time management skills to handle multiple cases and responsibilities effectively. Empathetic and compassionate attitude towards the elderly and disabled population. Proficiency in using computer systems and software for documentation and communication purposes.
