We're looking for an Aide to join our team.

You'll report directly to the Director of Operations and function under the direction, instruction and supervision of the Registered Nurse and/or Clinical Supervisor.

  • You'll perform assigned personal care activities and services to patient

  • You'll promote the patient's mental alertness through involvement in activities of interest

  • You'll provide basic emotional and psychological support to the patient and other members of the household

  • You'll report changes in the patient's mental or physical condition or home situation to the Registered Nurse and members of the interdisciplinary team

  • You'll provide accurate documentation with visit itineraries

About You

  • Compassionate, kind and empathetic to your patients and their families

  • Proof of certification in accordance with state law where hospice service is provided

  • Maintain good standing with state-specific Certified Nursing Assistant Board

  • Six months direct patient care experience in an institutional setting; one year preferred

VIEW JOBS 2/14/2020 12:00:00 AM 2020-05-14T00:00 Nature of Work This is specialized work in organizing, leading and instructing adaptive recreation at a recreation facility or community center. Work involves responsibility for organizing and leading a variety of adaptive recreation and sports activities for people with disabilities. Work also involves setting up equipment and facilities for sports, recreation and special events and instructing special needs participants. Work requires the use of tact and skill in dealing with participants with mental, physical, behavioral, and emotional disabilities while maintaining order and discipline. Work is performed under general supervision and is reviewed by a supervisor through conferences, activity reports and observation of performance. Assist in planning, implementing, scheduling, coordinating and promoting a variety of sports and adaptive recreation activities and programs; assists in planning and scheduling special events and social activities; assists in conducting camps, clinics and workshops. Teaching and conducting modified recreational classes and activities creating an environment that is inclusive and supportive to all participate with disabilities. Organizes, leads and coordinates adaptive recreation activities for youth, young adult, and adult groups; teaches and instructs participants; meets and confers with youth athletic league representatives, and oversees and monitors youth athletic activities; promotes good conduct, sportsmanship and safety; maintains order and discipline; issues and collects sports and recreation equipment. Provide high quality of customer service by consistently exhibiting an enthusiastic attitude. Acts as lead instructor/coach, and program assessment, including adaptive aquatics, sport camps, day camps, and weekly programs. Organizes and schedules arts and crafts and health and fitness activities. Assists in recruiting, training, monitoring and evaluating volunteer workers, under the supervision of a Neighborhood Services Programmer. Prepares activity and attendance reports. Assist with personal hygiene needs as needed. Provide one-to-one training or adaptive services to a specific participant. Promotes and maintains a cooperative relationship with public and private agencies in the neighborhood. Requires regular and prompt attendance plus the ability to work well with others and work well parents, caregivers, and special needs participates as well as other staff members. Provide excellent customer service to all program patrons by exemplifying a positive attitude and exceeding the needs of patrons. Assist with the implementation and supervision of a wide variety of populations and recreation activities in various seasonal programs. Encourage participants to be independent and support each participant unique learning style. Monitor participants to endure adherence to recreation facility standard and establish safety policies, procedures, guideline and operating requirements. Performs related work as required. Physical and Environmental Factors: Work is performed both indoors and outdoors. Work exposes employee to potential communicable diseases and bloodborne pathogens, inclement weather, physical hazards, and chemical hazards. Work requires frequent physical effort such as sitting, standing, walking, running, bending, stooping, kneeling, climbing and lifting light to moderately heavy objects. Work also requires physical dexterity in the frequent use of hand-eye coordination and manipulative skills using fingers, limbs or body in leading athletic activities and operating vehicles and equipment. Minimum Education, Training and Experience High school diploma from a school accredited by a regional accrediting agency recognized by the U.S. Department of Education or GED certificate issued by the appropriate state agency. College degree or equivalent combination of education and 2 years of related experience desired. Knowledge of adaptive recreational planning and supervision, and the rules and regulations governing various competitive athletic games. Knowledge of planning and implementation of recreational programs and services related to therapeutic, adaptive and inclusive reaction. Must be able to provide a structured play environment that meets the needs of all participants, being able to problem solve, work independently and have good communication skills. Ability to organize, schedule, lead and instruct adaptive recreational and social activities. Ability to communicate clearly and effectively, both orally and in writing, and to keep records and prepare reports. Ability to meet and deal tactfully with the public, and to establish and maintain effective relationships with superiors, other employees and volunteers. Ability to assess the recreational interests and needs of people with disabilities.
