Butler County and surrounding areas. This is a field position with a designated home office location and the incumbent is expected to travel when visiting patients in their homes.
Provides personal care to patients/clients under the direction of the nurse and/or as requested by the client.
Provides personal hygiene and grooming to patients/clients including but not limited to bathing, shampooing, nail care, skin care, oral hygiene, toileting and dressing.
Safely assists patients/clients to transfer, ambulate, reposition and perform range of motion exercises.
Performs simple treatments and procedures, like taking vital signs of patients/clients, as described in the plan of care.
Observes, reports and documents patient/client status and care provided.
Assists in maintaining a clean and healthful environment for patients/clients by performing simple household duties such as changing bed linens and separating dirty laundry from clean laundry.
Identifies areas which may present compliance problems with federal regulations for the position's assigned responsibilities.
Ability to drive personal vehicle independently in order to visit patients/clients in their home or a facility.
Performs other duties as assigned.
High school diploma or general education degree (GED);
At least two (2) years' experience as a nurse's aide in a hospital, skilled nursing facility, home health agency or as an independent provider in a person's home.
Working knowledge of computers, Microsoft Word and Excel required.
Successful completion of the Home Health Aide/Personal Attendant competency evaluation including a satisfactory rating in all skill areas and 85% or higher on the written examination.
