Lebanon County Area Agency on Aging is looking for a compassionate Aging Care Manager 2 to complete our team. We work to maximize individual functioning and optimal quality of life for all elderly with emphasis placed on the prevention of unnecessary and inappropriate institutionalization of older persons. This Care Manager will be responsible for the Pennsylvania Caregiver Support Program (PCSP) which links care giving adults with support and reimbursement (Financial) to assist in the care of care receiver. You will be receiving case assignments involving general casework and assessment procedures. Work involves interviewing and evaluating the consumers in their homes and developing a plan of care for the consumer. You will have the opportunity to provide counseling and support for consumers and their families to help them adjust to difficult situations and make them aware of the programs that our agency offers and how it may help solve some of their problems.

Work Schedule and Additional Information:

  • Full-time employment

  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.

  • Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Successful completion of the County Social Casework Intern program; or

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or

  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or

  • An equivalent combination of experience and training.

Other Requirements:
