Overview

We are looking for an Afterhours Clinical Support RN. This position reports to the VP Care Coordination and will provide supportive care to the patient and family after normal business hours on an on-call basis. This position will also manage and direct an interdisciplinary group consisting of Registered Nurses, Licensed Practical/Vocational Nurses, Hospice Aides and support staff to enhance the quality of hospice care.

• Maintains an accurate and updated medical record, including all Interdisciplinary notes, Plans of Care, Initial and on-going assessments and any other information pertinent to the care of the patient.

• Functions as the Administrator on-call throughout the weekend hours

• Performs the initial and on-going assessment of the patient as needed

• Helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the Interdisciplinary Group

About You

• Registered Nurse with minimum of Associate's Degree in Nursing

• Bachelor's degree in Nursing (BSN) from an accredited school of nursing preferred

• Current license to practice nursing in the state of work

• Minimum of 1 year experience in hospice

• Minimum of 1 year of management experience preferred
