Nexus-PATH is looking to add to our Treatment & Foster Care team! We will be adding an Aftercare Coordinator to support our Western-North Dakota Region (Minot, Williston, Dickinson, Bismarck, ND).
Position Summary:
Our Aftercare Coordinator prepares and facilitates a series of aftercare services activities, ensuring clients are seamlessly transitioned back into their family and/or community, during and after a foster care program placement.

Primary responsibilities:
Facilitate the completion of required communication and documentation surrounding transition/aftercare plans
Provide therapeutic services and support within the parameters of the program's structure, philosophy and models while emulating Nexus core ICARE values and mission
Serve as a role model for youth and clients, demonstrating appropriate social and emotional skillsets for youth and their caretakers; proactively support youth preparing for a smooth and healthy transition back to the community
Establish transparent, trusting and communicative relationships with the family, caregivers and youth to ensure their voice and choice is at the center of their aftercare plan and during their transition
Develop and facilitate family-centered transition and aftercare teams
Responsible for ensuring the transition/aftercare plans are documented, current, compliant and accessible starting at intake, maintained throughout the clinical intervention process and post discharge aftercare-up to six months
Creates community relationships to enhance resource and referral opportunities for the youth and their caretakers
Maintains state statute requirements through weekly contact with youth and families up to three months post Foster Care discharge
Maintains all client documentation in individual file
Provides intermittent transportation to clients and families as needed within reason
Regularly complies with all agencies, state and regulatory standards; documents compliance activities and communications as needed and required for clients within their file
Qualifications:
High School Diploma or GED equivalent
Some related field experience working with youth & adolescents
Valid driver's license required. Must meet state regulating agency and Nexus Home Office driving requirements
Nice to Have:
Bachelors or above in related field
Experience in youth & family treatment case management or aftercare setting
Experience in mental health or behavioral health setting
Travel:
Travel to home office and other sites occasionally as requested.
