Lakeview Health is looking for a professional, organized, and dynamic Aftercare Coordinator to join the Alumni team in Jacksonville, FL. This position would work closely with the Clinical and Alumni teams. The Aftercare Coordinator will work with the Clinical team in regards to patient care. They will be included within the treatment team and will coordinate with the Therapists, Case Manager, and Client Service Reps to coordinate aftercare for patients. This position will work with patients during their treatment and keep them engaged in their aftercare planning and will assist in deciding what will be the best options for them once they leave Lakeview Health or Stepping Stone.
The Aftercare Coordinator will work with the Client Service Reps in continuing to build relationships and partnerships with outside referral sources, resources, and facilities. This dynamic position will assess the different resources and determine what would be best for each patient based on: the patient's needs, insurance coverage, location, and the services provided by the resource. Keeping track of the patient's aftercare plan and following up with the referral sources, resources, and facilities would be imperative for this position.

Hours: Monday Friday, 8:00 am -5:00 pm (will be required to work evenings and/or weekends 4-5 days per month)
High school diploma, Bachelor's Degree strongly preferred.
At least 1 year of experience in Behavioral Health/Addictions.
At least 2 years in a Customer Service position demonstrating excellent communication and client relations skills.
Must have good follow through and be self-motivated with the ability to work independently and take initiative.
Knowledge of Lakeview Health Systems' program and awareness of the 12 Steps.
Experience in some case management or knowledge of outside resources preferred.
Must be extremely organized with strong attention to detail.
Must be computer proficient with experience with Microsoft Office products: Word, Excel, Outlook, etc.
