The Advanced Registered Nurse Practitioner (ARNP) within the Clinical Redesign Team provides in-home primary care in collaboration with a behavioral health clinician to chronically ill and high risk patients in a variety of environments including private homes, and may include assisted living facilities, nursing homes, skilled nursing facilities, a clinic, and/or hospital settings. The ARNP exercises autonomy in medical decision making while collaborating with a multidisciplinary team to engage resources and strategies to address medical, functional, and social barriers to care.

Manage a patient panel with a daily schedule of multiple patient visits for complex primary care, post-hospital discharge stabilization, and follow up
Evaluate and document accurate patient histories to provide expert assessment and treatment of complex health conditions in a variety of settings: In-home visits, hospital visits, and phone encounters
Review, interpret, and integrate data to determine appropriate diagnostic and therapeutic procedures with the goals of reducing readmissions, avoiding nursing home placement when home care is possible and more dignified, and decreasing future avoidable medical system utilization and therefore cost
Develop plans of care using established trauma informed and person centered principles, demonstrating expertise in caring for individuals with complex needs
Manage medical emergencies, triage incoming calls, and prescribe medications
Assess level of functioning and appropriate care plan (treatment plan) goals and objectives
Collaborate with a multidisciplinary team to engage resources and strategies to address medical, functional, and social barriers to care
Provide diagnostic impressions and collaborate with other specialists of care providers for coordinated interventions to address symptomology
Assess risk levels and develop associated safety/wellness plans
Counsel and educate patients and families about acute and chronic conditions or concerns
Complete full and appropriate documentation of patient care in order to support professional billing for these services
Participate in rotating on-call rotation for similar care teams
Support matrix partners to ensure the business unit remains compliant with all clinical best-practices, regulatory requirements, and contract requirements
Collaborate with staff/leadership team to decrease unnecessary healthcare utilization and increase beneficial, cost-effective utilization using evidenced-based strategies
Clinical Redesign is a business unit focused on developing and deploying innovative direct care models for underserved populations. The business unit is housed within Community & State, the Dual Special Needs insurance arm of UnitedHealthcare. The Clinical Redesign Team is deploying a range of direct care models targeting both complex and healthy populations including members with medical, mental health, addiction, and social complexity with a trauma-informed approach. As the R&D, innovation team within Community & State, the business unit is charged with transforming the way care is delivered, to achieve better care at lower cost. The team also draws upon a range of business best practices to ensure the services will scale and spread with strong fidelity and high quality service delivery.

Residence within the identified geographic territory for the program
Graduate of an accredited Master's Degree in Nursing (MSN) program
Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)
1 years' experience delivering primary care in a clinic environment
3 years' fulltime professional experience as an APN
Unrestricted nursing license in good standing through the applicable state of hire
2 years' experience caring for both medically and socially complex populations and Medicaid and Medicare beneficiaries
2 years of experience working with SPMI/SMI population and individuals with substance use
3 years' experience providing in person, in home (field based), or virtual care
3 years of professional experience using technology including laptop, tablet, smart phone, Microsoft Outlook, electronic medical record, and various virtual business platforms
Current DEA licensure with DATA-waiver (X-waiver)
Home care or home visit experience
Suicide assessment/prevention experience
Knowledge of palliative care philosophy and approach; familiarity with end of life care; hospice experience
Expertise in motivational interviewing
Knowledge of health benefits for DSNP and Medicaid in the assigned market
Familiar with the principles of trauma-informed care and engagement that embodies these principles
Previous experience with E-Clinical Works electronic medical record
Experience developing Wellness and Recovery Action Plans (WRAP)
