Summary The Advanced Practice Registered Nurse practices medicine under a collaborative agreement with a departmental physician, with both disciplines practicing within the scope and boundaries of State regulations. Provides reasonable and prudent medical/surgical care, practices appropriate management, and uses sound clinical judgment based on best practice standards and evidenced based medicine.
Provides general medical care, patient education and treatment to patients of CS-HHC. Promotes and maintains health by providing services under the supervision of the Department Chief/Physician. # CS-HHC#s Expectations of all Employees # Adheres to all CS-HHC Policies and Procedures Conducts self in a manner that represents CS-HHC#s Values at all times Maintains a positive and respectful attitude with all work-related contacts Communicates regularly with supervisor about Departmental and CS-HHC concerns Consistently reports to work on time, prepared to perform the duties of the position Meets productivity standards and performs duties as workload necessitates # Essential Duties and Responsibilities # Provides reasonable and prudent medical/surgical care, practices appropriate management, and uses sound clinical judgment based on best practice standards and evidenced based medicine Maintains confidentiality in regards to patients, staff and program matters at all times Consults with other staff or specialists to make appropriate referrals for acute, chronic and preventive health care, as may be indicated Coordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary Educates patients on preventive health care and chronic disease management Treats patients and staff respectfully, courteously, appropriately Maintains safe and clean working environment by complying with procedures, rules and regulations Protects clients and employees by adhering to infection-control policies and protocols Completes charting of notes, forms and coding in a timely and efficient manner. Enters appropriate legible notations, documents encounters appropriately Attends/participates and contributes to medical staff/departmental meetings Participates in trainings as required by the Department Chief Performs other related duties as assigned # Qualifications
