Advanced Practice Registered Nurse (Aprn)

Choate Rosemary Hall (CRH) is seeking a pediatric or family nurse practitioner to provide primary care to students. This is a unique opportunity to join an experienced clinical team in an adolescent friendly clinic with young people from across the country and around the world. We take a developmental, trauma-informed approach too primary care and work closely with our integrated counseling team, athletic trainers, and campus health educators. Duties and Responsibilities

• Provides primary medical care for all students.

• Develops a plan of care for each student, including medical and social history, physical examination, diagnosis and treatment and/or referral including hospitalization if necessary.

• Uses all available resources in diagnosis and treatment, such as laboratory and other tests in a thoughtful, timely and cost-efficient manner.

• Confers with school nurses, other clinicians, families, admin, faculty, coaches, advisors and support staff concerning treatment and care of student.

• Works closely with Counseling Team and Athletic Trainers on campus to provide integrated and comprehensive primary care services.

• Refers to off-campus specialist services but maintains primary care responsibility to assure close follow-up and continuity of care.

• Works closely with students' home-based clinical team to assure continuity of care.

• Maintains thorough and up to date medical records and performs timely and accurate charting.

• Maintains confidentiality.

• Participates in medical and general staff meetings and clinical teams.

• Participates in Quality Assurance and Performance Improvement of health center as well as Peer Review and in other task groups as needed.

• Pursues continuing education opportunities to update medical knowledge and assure quality patient care.

• Performs other necessary duties as required by CRH to further the mission of the school and to achieve the goal of providing high quality service and health care.

• Graduate of accredited school of nursing, with master's level preparation.

• Certified or eligible for certification by the appropriate National Certifying Organization.

• Licensed by the State of Connecticut as an Advanced Practice Registered Nurse.

• Qualified in Basic Life Support techniques.

• Multi-lingual desirable, not mandatory.

• Experience in school-based/community/outpatient setting.

• Ability to relate with warmth and effectiveness to students, staff, faculty and families.
