Hartford Hospital is seeking a full-time, experienced Nurse Practitioner or Physician Assistant to work as the Advanced Practice Quality Coordinator of the Department of Surgery. The Quality Coordinator will be responsible for the advancement of best practice standards, continuous quality improvement and system efficiencies within the Department of Surgery. The Quality Coordinator will be responsible for effectively identifying and translating successful quality and safety processes and standards, and mandates into sustainable daily practice and operations. This position requires integration of clinical expertise and quality standards across the care continuum. This role creates an environment that promotes professional practice, and strives for excellence in all aspects of clinical interdisciplinary care within the Divisions of the Department of Surgery.

Role: Open to Nurse Practitioners (APRN) and Physician Assistants with 5yrs min clinical experience

Location: Hartford Hospital Department of Surgery (on site)

Setting: Inpatient and Administrative

Advanced Practice Quality Coordinator Role Scope:

Responsible for the advancement of best practice standards, continuous quality improvement and system efficiencies within the Department of Surgery. Responsible for effectively identifying and translating successful quality and safety processes and standards, and mandates into sustainable daily practice and operations. This position requires integration of clinical expertise and quality standards across the care continuum. This role creates an environment that promotes professional practice, and strives for excellence in all aspects of clinical interdisciplinary care within the Divisions of the Department of Surgery.

  • Establishes a structure to mitigate patient safety risks by incorporating best practices, decision support alerts, and safety reporting culture

  • In partnership with Leadership team, establishes standards for use of data to monitor the outcomes of care processes and integrates improvement methods to design and test change to improve the overall quality and safety of the Divisions of the Department of Surgery

  • Collaborates in the identification of gaps in processes and procedures that are inconsistent with current standards and best practices

  • Integrates and disseminates evidence-based research into educational offerings, quality projects and potential research

  • Provides a vehicle to share metrics of both process and outcomes with front line staff

  • Provides metrics and reports to review processes and outcomes regularly

  • Value based care and cost of care is integrated into daily practice

  • Works with system quality and safety leaders to ensure that the Divisions of the Department of Surgery are reflective of safe and best practices through case reviews, RisKonnect / event reporting

  • Supports an evolving framework to support safety and quality improvement culture in alignment with HHC Balanced Scorecard

  • Reduces preventable patient harm, increase efficiencies and improve the value in healthcare

  • Coordinates and supports the integration of evidence-based practice (EBP) within the interdisciplinary team

  • Identifies policies, procedure or pathway structures for practice, standardization and process change

  • Acts as a resource across the care continuum, sharing current literature and research and best practice standards

  • Acts as a mentor for the Division Specific Advanced Practice Quality Coordinators in the Department of Surgery

Qualifications

Minimum Requirements:

  • Bachelor's Degree in Advanced Practice such as Nurse Practitioner/Physician Assistant

  • Minimum 5 years of experience in an acute care setting

  • Current Nurse Practitioner or Physician Assistant w/ CT license

  • Ability to promote system integration within a large health system

  • Demonstrated knowledge of clinical integration and evidence-based practice and standards of care

  • Ability to provide leadership in the transformation of quality and safety

  • Ability to mentor colleagues from across the inter-professional spectrum

Preferred Requirements:

  • Master's Degree in health care
