Clinical Responsibilities:
The Advanced Practice Provider requires broad clinical knowledge and experience in palliative care.
Mentor physicians and nurses, supporting the primary care team.
Conduct medical and psychosocial evaluations of patients and families.
Active presence in multiple ICUs, implementing palliative care protocols.
Develop and apply screening processes for ICU palliative consults.
Lead discussions on care goals with ICU teams.
Independently follow up on patient consultations and symptom management.
Participate in ICU rounds, manage the Palliative Care ICU consult pager.
Collaborate with the interdisciplinary palliative care team.
Conduct consults beyond ICU if required, including LVAD/Heart Transplant evaluations.
Daily data entry into Palliative Care Spreadsheet.
Educational Responsibilities:
Develop policies, procedures, and protocols.
Present in-service sessions on Palliative Care.
Train students, residents, and fellows on the Palliative Care consult service.
Precept Advance Practice Nurse students.
Additional Activities:
Attend daily Palliative Care Interdisciplinary team meetings.
Regular meetings with Palliative Medicine Medical Director.
Maintain Palliative Care continuing education.
Participation in nursing department committees.
Lead or participate in special projects as needed.
Position Requirements:
Minimum Education:
Required: Master's Degree

o CPR - Basic Life Support
o Nursing
Current Registered Nurse License
State of Illinois
o Nursing
National advanced practice certification in adult, geriatrics, or family practice is required
State of Illinois
It is expected that each nurse practitioner will become an Advanced Certified Hospice and Palliative Nurse within one year of the date of hire.
