Summary:

The Nurse Practitioner/Physician Assistant is accountable to facilitate a quality hospital experience and outcome for patients within a highly specialized team in Neurointerventional Radiology and Vascular Neurology. The Nurse Practitioner/Physician Assistant will be responsible to the Medical Director of Patient Services and responsible for overseeing the progress of a specific caseload of patients across the care continuum. Acts as an educator consultant and liaison to healthcare providers caring for the patients across the care continuum. The Advanced Practice provider (APP) hired for the Neurovascular Center (NVC) will be an integral part of the team caring for ischemic and hemorrhagic stroke patients as well as other cerebrovascular disease. Assists in the care of these patients throughout their hospitalization including the hyperacute phase. The work performed will primarily be in three locations: The Emergency Department Stroke Service and in Neurointerventional Radiology.

Responsibilities:

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Demonstrates a fundamental knowledge in the field. Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings. Oversees care coordination in acute home care and ambulatory care settings as feasible. Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support education and encouragement. Demonstrates advanced physical and psychosocial

assessment skills and knowledge of family dynamics. Provides direct care to patients in acute care care settings. Performs advanced practice skills under the supervision of the Medical Director. Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee. Participates in the education clinical research and mentoring of healthcare providers. Facilitates patient/family education in area of expertise. Annually completes mandatory safety infection control BLS and ACLS/PALS. Prescriptive privileges per licensure.

Emergency Department � Hyperacute patients

  • Responds to all code Strokes (in patient and ED) and understands the role of acute revascularization therapies (IV and IA) in the care of these patients.

  • Obtains and maintains NIHSS certification

  • Obtains and maintains certification for Modified Rankin Scores

  • Communicates and coordinates the hyperacute care of

these patients with ED Neurology & NIR

  • Obtains consent and performs pre-procedure evaluation if patient is going to NIR

  • Off hours begin NIR preparing room for the procedure.

  • APP will scrub and participate in the procedure. This should include other aspects of the VIR case as listed below.

  • Assists Neurology resident with admission H&P orders

if needed.

  • Assists in hyper-acute MRI process when indicated.

Stroke Unit - Ischemic and Hemorrhagic stroke patients

on Stroke Unit (and their ED Evaluation)

  • Rounds with Vascular neurology team including performing H&P daily exams progress notes discharge summaries and hospital course.

  • Acts as resource to Neurology resident with other issues on Stroke service patients as needed NIR Service � Prior to During and after neurointerventional procedures

  • Assist NIR attending with consent pre-procedure H&P discharge summaries discharge instructions for outpatients and any other pre- intra- or post-procedural issues as needed.

  • Become proficient at the following aspects of neurointerventional procedures:

  • Understand sterile technique

  • Understand radiation safety and using fluoroscopic equipment safely

  • Prepare sterile tray and understand process of preparing wires catheters balloons and other devices as indicated

  • Obtain arterial and venous access including safe

placement of sheaths into tortuous anatomy

  • Assist primary operator in interventional cases including understanding of catheter and wire manipulation in cases

  • Proficiency at arterial and venous access closure including the use of arterial closure devices.

Other information:

BASIC KNOWLEDGE:

Masters of Science Degree Graduate of accredited Nurse Practitioner or Physician Assistant program. Specialty certification required for Nurse Practitioners.

Baccalaureate in Nursing

Certified in Basic Cardiac Life Support (BLS) and Advanced Cardiac Life Support (ACLS).

EXPERIENCE:

One to two years related experience as an APP in acute care hospital setting and preferred

INDEPENDENT ACTION:

Performs independently within the department�s policies and practices. Refers specific complex problems to the supervisor when clarification of the departmental policies are required.

SUPERVISORY RESPONSIBILITY:

None.
