Responsibilities*

Under physician supervision, coordinates multidisciplinary care of aortic disease patients admitted to the MI-AORTA Program. Specifically, role requires the clinician to:

Perform consultations, pre-op history and physical exam, and follow up with the patients in the aortic disease program, including, but not limited to, placing orders, following and reviewing pre-procedures testing to ensure complete evaluation.

Collaborate with other care providers/consultants to coordinate and communicate clinical care across health system to provide aortic patients with a single care check point and navigate their next care steps.

Perform inpatient consultations and collaborate with inpatient clinical team/Ambulatory CCC to facilitate transitions of care.

Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, the RN/Clinical Team; streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external Michigan Medicine team members and other ambulatory care providers.

Educate patients and their families on disease management, information and details regarding surgical procedures, post-operative instructions, and preventive care.

Participate in clinical trialuired to assure patient safety and satisfaction.

Participate in the development and presentation of educational experiences for patients, families and health care professionals.

Work with administrative director for MI-AORTA program to create and implement strategies for programmatic growth.

Work with administrative director for MI-AORTA program to determine and track programmatic outcome metrics; collect and analyze outcome metrics for programmatic improvements.

Incorporate research finding into clinical practice and integrate quality assurance into practice improvements to facilitate programmatic growth.

Participate in and present patients at regular case presentation meetings.

Collaborate with physicians for addressing all incoming patients calls and collaborate with the FCVC Contact center.

Maintain an in-depth knowledge of medical principles, practices, standards, and techniques in accordance with Michigan Medicine policies and procedures, Medical Staff Bylaws, and State and Federal laws/regulations.

Utilize Service Excellence and Patient Family Centered Care in daily practice.

Abide by the standards of care and standards of practice established by Michigan Medicine.

Practice within the regulations of the Michigan Public Health Code.

: This is a 1.0 FTE full time position, primarily days, variable shift with potential evenings/weekends. Hybrid remote work based on clinical needs of program.

Nursing Specific Info

Employment is contingent upon successful completion of the credentialing and privileging process.

Required Qualifications*

NP:

  • Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.

  • Current specialty certification to practice as an Adult-Gerontology Acute Care Nurse Practitioner.

  • Graduate of an accredited Nurse Practitioner program.

  • Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).

  • Current DEA registration required.

  • ACLS required (separate AHA approved BLS also required unless ACLS was completed through the UMHS Simulation Center).

PA:

  • Current license to practice as a Physician Assistant in the State of Michigan.

  • Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.

  • Certification by the National Commission on Certification for Physician Assistants (NCCPA).

  • State of Michigan Controlled Substance License Required.

  • Current DEA registration required.

  • ACLS required (separate AHA approved BLS also required unless ACLS was completed through the UMHS Simulation Center).

Desired Qualifications*

  • Demonstrated experience in the management of complex aortic patients including aortic dissection patients.

  • Demonstrated experience serving as a liaison for the program to referring providers.

  • At least 3 years working full time in an Advanced Practice role in cardiac surgery or cardiology in the last 5 years.

  • 3 years experience within the last 8 years caring for the cardiac surgery population.

  • 4 years experience within the last 8 years caring for cardiology patients.

  • Specific experience as an Advanced Practice Provider caring for patients with valvular disease, aortic pathology.

  • Minimum of 1 year experience caring for pediatric population including experience with pediatric aortic disease including abdominal aortic coarctation and aortic or branch aortic stenotic/aneurysmal disease.

  • Demonstrated experience in the management of complex aortic patients.

  • Experience with wound care management.

  • Demonstrated experience in caring for patients across the continuum of care.

  • Experience functioning as an independent provider in addition to contributing to a team environment, working efficiently, and experience independently managing high acuity patients.

  • Advanced understanding of cardiac surgery procedures, including an ability to explain procedures in detail to patients and their families.

  • Demonstrated ability to order and interpret appropriate diagnostic studies; synthesize information and make informed clinical decisions independently; develop appropriate care plans, ability to be patient-focused and respond to patient and family preferences and needs.

  • Demonstrated knowledge of adult cardiac physiology and complex cardiac pathology with experience teaching cardiac physiology and pathology in a high level learning environment.

  • At least one year of recent experience caring for acutely ill patients in a hospital setting.

  • Demonstrated excellent verbal and written communication skills, organization, and problem-solving skills.

  • Demonstrated high level of critical thinking.

  • Ability to prioritize assignments and manage a busy work environment.

  • Demonstrated high level of motivation to learn within and outside of the hospital environment and to participate in activities and projects to improve patient care and outcomes.

  • Must reflect a professional appearance and demeanor and possess excellent customer service skills.

  • Demonstrated commitment to carrying out professional responsibilities, adherence to ethical principles, and sensitivity to diversity.

  • Ability to demonstrate computer skills including computer documentation and order entry.

  • Demonstrated experience with and knowledge of current medical, surgical, and pharmaceutical treatment modalities of the cardiothoracic surgery patients.

  • Demonstrated expertise in facilitating the educational growth of others through previous instructional or mentoring roles.

  • Focus on research including clinical trial participation opportunities.

  • Focus on patient and family-centered care.

This position is covered under the collective bargaining agreement between the U-M and the Michigan Nurses Association and the U-M Professional Nurse Council union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
