Provides coordination/case management for advanced heart failure patient population. Works in partnership with multidisciplinary team members to ensure care coordination of clinical treatment.

Effectively uses transplant committee as an opportunity for care conference regarding difficult cases. Provides professional, peer and patient education.

Shift Details: Average 40 hours weekly, Full-time exempt position.

Required qualifications:

• Bachelor's degree in nursing

• Washington Registered Nurse License upon hire

• 5 years Acute, Ambulatory or Clinic nursing experience

Preferred qualifications:

• 2 years recent experience in, cardiology, critical care or transplant nursing
