Summary Advanced Care Partner (ACP)

Under the supervision of an RN, performs basic patient care activities. See Advanced Care Partner Program WEBSITE for complete Program explanation.

Job Duties

  • Performs basic patient care activities as delegated and supervised by an RN, including but not limited to, monitoring vital signs, taking blood glucose measurements, and recording intake and output.

  • Assists patients with tending to personal care and activities of daily living, including but not limited to, bathing, grooming and eating.

  • Reports abnormal findings or changes in physical, mental and emotional conditions to nursing staff.

  • Assists with keeping unit(s) and patient rooms stocked, clean, and orderly.


  • Enrolled in an accredited BSN program, without interruption and in good standing.

  • Must have successfully completed Med/Surg 1 at time of application with the ability to provide transcript with grade.

  • BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire. All life support certification cards must be issued by either the American Heart Association or the American Red Cross CPR for the Professional Rescuer. (American Red Cross for the Professional Rescuer is only good upon hire and all renewals will need to be through an American Heart Association sanctioned course.)

Under the supervision of the Pharmacy Director and/or the Assistant Director of Pharmacy, the Advanced Clinical Pharmacist is responsible for the safe, effective and cost-effective selection and utilization of medications for the patients they serve. The Advanced Clinical Pharmacist uses advanced knowledge, skills and professional judgement in all aspects of their work. The Advanced Clinical Pharmacist duties include but are not limited to, attending and actively participating in daily multidisciplinary rounds, providing clinical consultations and clarifications to practitioners, participating in data collection of drug utilization evaluations and pharmacy quality initiatives, participating in various nursing and medical staff committees, providing staff education in the area of expertise, mentoring pharmacy students and residents, responding to Code Blues and various alerts, and maintaining effective pharmacy operations. When need arises, the Advanced Clinical Pharmacist will perform other duties as assigned by supervisor. The Advanced Clinical Pharmacist will have additional leadership responsibilities (service line development, committee involvement, educational development, etc.) pursuant to clinical specialty. A. Licensure/Certification/Registration: Current licensure by the Colorado Board of Pharmacy as a registered Pharmacist upon hire date. BCLS and ACLS certifications within first 12 months of employment. Must maintain current certifications. B. Education: BS or PharmD in Pharmacy required. Board Certification preferred but not required. C. Experience: Minimum of PGY1 Residency with at least one year post-residency clinical experience OR three years of hospital clinical pharmacy experience. D. Special Qualifications: Must have the ability to type, must have computer skills and must have effective oral and written communication skills. The candidate must demonstrate skills commensurate with formal training. Degree of supervision required: Involves general guidance and direction by the Pharmacy Director. Employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures. Population and Ages of Patients Served: The Advanced Clinical Pharmacist serves all types of patient populations (neonatal, pediatric, adolescent, adult, and geriatric) and disease states. This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards.
