This is a twelve month Advanced Practitioner post graduate fellowship program focused on intense Critical Care evidence based didactic and clinical education. The program is ideal for a PA/ACNP graduate seeking a structured educational program with the goal of transitioning from student to a skilled and successful Advanced Practitioner (AP) or for those looking to build more experience in managing complex critically ill patients.

Building upon the AP?s medical education, this fellowship will enable Advanced Practitioners to become highly functional and essential members of a Critical Care team. Applicants should be highly motivated, confident and patient centric team players who have a strong interest in Critical Care as their chosen profession. Motivated learning with time management and adaptability skills are highly desirable.

The AP Fellow will primary rotate through medical intensive care units in both the tertiary care and community settings, and will have exposure to subspecialties including nephrology, ID, neurocritical care, and others. The AP Fellow will be expected to learn and become proficient in critical care procedures, including, central lines and arterial lines, with the possibility of exposure to additional procedures such as paracentesis, thoracentesis, chest tubes and intubations.

VIEW JOBS 1/3/2019 12:00:00 AM 2019-04-03T00:00 GENERAL SUMMARY / OVERVIEW: Work directly with patients, Specialty Pharmacy Clinical Coordinators, and clinic staff to manage the specialty medication needs of Partners HealthCare patients. Understand the complex medication needs of patients and coordinate with pharmacy and medical/nursing staff to meet those needs while providing extraordinary service. Assist patients with benefit coordination and financial assistance, manage prior authorizations, and ensure patients receive their medications when needed. PRINCIPAL DUTIES AND RESPONSIBILITIES: Work with physicians, nurses and pharmacists to ensure that each patient receives the medications he/she needs at the appropriate time: * Review medication regime with patient to determine which medications need to be dispensed * Process medication orders confirming that they are billed to all third parties correctly * Monitor patient concerns, charts and profiles to meet their medication therapy needs Assist in education efforts for PHSP: * Answer patient questions about the service, insurance issues, etc. * Ensure all questions are answered timely and accurately * Provide patient education materials and information about other resources Commit to Building Relationships: * Quickly forge and sustain (or develop and maintain) strong relationships with pharmacists, co-workers, prescribers, nurses and patients to build the service * Provide caring service, adjusting approaches to reflect developmental level and cultural differences of population served * Improve processes to facilitate communication and service between patient, and clinic, infusion center and discharge pharmacy teams to ensure accurate and timely delivery of medications Act as the insurance specialist: * Understand Medicare/Medicaid regulations and billing guidelines as they pertain patient pharmacy benefits * Understand billing primary, secondary and tertiary insurance claims * Resolve any billing issues and obtain Prior Authorizations for medications. * Obtain benefit information and verify patient insurance eligibility and level of coverage Assist in the preparation and dispensing of medications and supplies as assigned: * Receive prescriptions from patients and clinicians; gather customer information per department policy. * Prepare prescription drug therapy for patients in an accurate and timely fashion in accordance with all legal requirements, regulatory agencies and Partners Specialty Pharmacy Policy * Assist the pharmacist in identifying patient problems and issues * Package medications as necessary to ensure safe handling and appropriate temperature control in mail service delivery. Complete all documentation necessary for tracking purposes: * Ship medications supplies and equipment to patients, or other areas as assigned Assist in maintenance of the pharmacy to reflect a clean and well-stocked condition daily: * Ensure each product has a POS shelf label Process prescription information in the computer system: * Help in the maintenance of pharmacy billing records * Assist in complete and accurate billing of third party pay plans Assist in inventory control functions: * Maintain stock of inventory in assigned areas * Receive, unpack, and check incoming stock against invoices and put into inventory * Process recalls and returns when necessary * Help in the maintenance of pharmacy billing records Complete the work of a pharmacy technician as required: * Cross train and cover all department technician areas assigned * Assist in maintenance of the pharmacy to reflect a clean and well stocked condition daily * Provide 24/7 on-call support on a rotating basis with other Specialty Pharmacy team members to address urgent issues that arise outside of normal business hours Provide superior phone service to customers and patients: * Speak with patients on both incoming and outgoing phone calls * Problem solve and triage patient phone calls to pharmacist as necessary Assist Clinical Coordinators with tasks as assigned: * Collect data and run statistics for internal and external reporting Demonstrates Commitment to Customers: * Seeks to understand and value differences between employees * Treats other employees with dignity and respect * Demonstrates open, honest communication and behavior * Resolves conflict in a positive manner * Contributes to the productivity of the workgroup * Use/s the Partners HealthCare values to govern decisions, actions and behaviors.
