This position works in collaboration with a physician in active practice to provide comprehensive, quality hospice and/or palliative care services to patients. The position may focus on solely on palliative or hospice care; or may be a blended role of palliative care and hospice care. This position may be focused in adult and/or pediatric care. The advance practice provider practices with a physician who is ultimately responsible. This provider:

  • proactively identifies opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resource use
  • builds effective relationships with referral sources and physicians to ensure excellent team follow-up regarding patient consults and to build an improved culture of collaboration among all providers to meet the needs of the palliative care and/or hospice program's patients and families

Description of Job Responsibilities:
1.Palliative Care Responsibilities

i) Utilizes approved clinical guidelines and evidence based protocols when providing care for palliative care patients and families

ii) Perform comprehensive and focused evaluations of patients referred for palliative care consults and/or on service

iii) Identifies emergent and ongoing physical, emotional, social and spiritual symptoms and determine executive, cognitive and functional capacity of patient and caregiver.

iv) Assesses disease and treatment burden asking questions designed to elicit patient and family goals and promote advance care planning

v) Performs medication reconciliation and optimization, including discontinuation of non-beneficial medications when appropriate

vi) Assists with diagnosis, treatment and management of acute and chronic health conditions within scope of practice

vii) Order, perform and interpret clinical tests and prescribe medications including controlled substances as outlined in departmental procedures and protocols and as approved by the NC Joint Committee of the Board of Nursing and Board of Medical Examiners.

viii) Consults with medical director, other attending physicians and collaborating physician(s) as needed

ix) Collaborates with home health nurse, hospital nurse, attending and palliative care physicians and others as needed to identified patient needs and order medication adjustments, treatments and other care

x) Actively participates in weekly interdisciplinary team meetings

xi) Preforms administrative duties such as reviewing patient charts and procedures to provide input to patient information management system.

xii) Assists with conferences and rounds.

xiii) Coordinates scheduling of tests.

xiv) Collaborates with physicians, house staff, and nurses in development of protocols, manages protocols.

xv) Performs educational and research activities, assists in orientation and instruction of medical students, house staff and other clinician, assesses educational needs of patients identifies clinical problems participates in research activities participates in Quality Assurance projects for respective department.

xvi) Plans patient care which may include entire course of treatment for patient, ensures continuity of care and plans for discharge. Obtains medical history and performs physical examinations, performs diagnostic tests.

xvii) Provides patient care, e.g., schedules surgical procedures assists in OR induces IV infusions obtains lab studies and specimens manages wound care directs equipment use places prostheses as needed.

xviii) Initiates reimbursement for services rendered as appropriate.

xix) Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided
2.Hospice Responsibilities

i) Provides medical expertise on pain, symptom management and evaluation of terminal prognosis to patient, family members and staff

ii) Ensures that care being provided is based on established hospice care guidelines and standards.

iii) Utilizes approved clinical guidelines and evidence based protocols when providing care for hospice patients and families

iv) Perform comprehensive and focused evaluations of patients referred for consults and/or on service

v) Identifies emergent and ongoing physical, emotional, social and spiritual symptoms and determine executive, cognitive and functional capacity of patient and caregiver.

vi) Performs medication reconciliation and optimization, including discontinuation of non-beneficial medications when appropriate

vii) Prescribe medications including controlled substances as outlined in departmental procedures and protocols and as approved by the NC Joint Committee of the Board of Nursing and Board of Medical Examiners.

xviii) Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided

xix) Performs patient visits in private residences, long-term care facilities, assisted living facilities, hospice inpatient unit and other locations.

xx) Acts as the attending physician for identified hospice patients.

xxi) Communicates and collaborates with palliative care team to identify new patient referrals

xxii) Provides hospice medical director with face to face encounter documentation including:

(1) Brief narrative explanation of clinical findings that support life expectancy of six months or less.

(2) Signed/dated face to face encounter attestation from including date of encounter and attestation of information collected during encounter.

Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program in good standing.

Licensed as an Advanced Practice Provider (i.e., Nurse Practitioner, Physician Assistant, Clinical Pharmacist Practitioner, Certified Nurse Midwife, and Certified Registered Nurse Anesthetist) in the state of North Carolina. Board certification if required by the Department.

Minimum of two years' experience as RN or NP with hospice or palliative care experience preferred

  • Demonstrated clinical competency.

  • Effective leadership skills.

  • Strong interpersonal skills and ability to work effectively with a wide range of constituencies in a diverse community

  • Ability to work independently and collaboratively.

  • Demonstrated written and verbal communication and strong analytical skills.

  • Effective interaction to ensure the successful care of patients in the clinical setting.

  • Documentation in medical record to substantiate accurate billing.

  • Knowledge of Medicare "Face to Face" encounter criteria (hospice)

  • Current state driver's license and automobile insurance

  • Ability to provide on-call coverage

  • Performs other duties as assigned by Medical Director

All work performed is under the direct supervision of a registered nurse or physician. Description of Job Responsibilities: * Assist physicians and other health care providers with routine examinations and procedures, as well as procurement of equipment and supplies. * Provide administrative and clerical support including PI audits and data entry * Provide clinical support assistance to include, but not limited to, cleaning the work area, maintaining office equipment, troubleshooting common software and hardware problems, changing linens, checking supply inventory, re-stocking supplies, transporting patients/ supplies/ specimens, and assisting in the orientation of new employees and students. * Provide direct personal care to patients as outlined in plan of care activities such as bathing, dressing, feeding, assisting with ambulation.
