Anderson Transitions Clinic

Ascension Medical Group St. Vincent is currently seeking an Adult Nurse Practitioner for our Transitions Clinic in Anderson, IN. The providers on the transitional care team specialize in post-acute care medicine in an outpatient setting, with the goal of helping each patient make a healthy transition from hospital to home to reduce hospital re-admissions and promote recovery.

This candidate will be working M-F 8-hour days with no call. We are seeking a candidate who is comfortable working independently and who has acute care/critical care experience. We are not accepting new grads at this time.

What You Will Need


  • Required Credential(s):

  • BLS Provider obtained within 6 Months (180 days) of hire date or job transfer date.

  • One or more of the following:

  • Nurse Practitioner credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date.

  • Nurse Practitioner credentialed from the National Certification Corporation (NCC) obtained within 6 Months (180 days) of hire date or job transfer date.

  • Nurse Practitioner credentialed from the American Nurses Credentialing Center (ANCC) obtained within 6 Months (180 days) of hire date or job transfer date.

  • Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date.

  • Registered Nurse credentialed from the American Nurses Credentialing Center (ANCC) obtained prior to hire date or job transfer date.


  • Diploma

  • Master's degree of Nursing required

Work Experience:

  • At least 1 year of experience required

Job Summary: The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides psychiatric care including the assessment, diagnosis, and treatment of acute and chronic health conditions.
