About the Job

Case Management is defined in State Statute as providing coordination and access to services required by people experiencing serious and persistent mental illness (SPMI, adults) or serious emotional disturbance. The clinic has a contract with Hennepin County and various Managed Care Organizations to provide these services to eligible patients who reside in Hennepin County. The scope of the work and the requirements for supervision of the work is found in MN Statute and in the Health Care Provider Manual published by the Department of Human Services.

Reporting Relationships:

This position reports to a Behavioral Health Program Manager.

Essential Functions & Percentage for Each Function:

1.Provide adult case management services to clients in the community and at the clinic (85%)

Assessment:

  • Collect information to identify the recipient's functioning level and need for services.

  • Review and complete functional assessment to determine strengths and impairments.

  • Routinely contact, monitor or communicate with the recipient, family members, providers, or other relevant persons about the status of the recipient, the individual service plan or progress in achieving the goals of the service plan.

  • Maintain records related to the recipient's status and functioning.

Planning:

  • Develop a service plan (Individual Community Support Plan) with the client (and family or care team) designed to meet the needs of the client which were identified in the assessment.

  • Develop and revise goals as needed including written service agreements and routine case supervisory activities.

  • Receive clinical supervision for identified recipients with serious and persistent mental illness (SPMI).

Referral, Coordination, Monitoring:

  • Make effective linkages with natural and social supports as outlined in the treatment plan.

  • Monitor client use of services to assure that services are meeting client needs as planned.

  • Facilitate effective referrals providing support to the client as needed; including arranging and/or providing transportation to clients when appropriate.

Working Relationship with Client:

  • Develop relationships and trust in order to support the client in achieving their goals.

  • Demonstrate ability to engage clients and meet with each client at least once a month.

2.Participate in team meetings and individual supervision (15%)

  • Attend and participate in team meetings.

  • Complete charts and notes within 24 hours of client meeting.

  • Meet productivity and documentation/charting standards.

Required Qualifications:

Education /Experience

  • Holds a BA/BS in one of the behavioral sciences or related fields including but not limited to social work, psychology, or nursing from an accredited college or university.

  • At least 2,000 hours of supervised experience in the delivery of services to persons with mental illness.

  • Without a Bachelor's Degree meets one of the following:

  • Has 3 or 4 years of experience as a case manager associate.

  • Registered nurse and have a combination of specialized training in psychiatry and work experience consisting of community interaction and involvement or community discharge planning in a mental health setting totaling three years.

Licensure Requirements:

  • Licensure is not required unless you hold a Social Work degree (BA, BA, or MA).
  • A Social Work license (LGS, LGSW, LISW, or LICSW) is required only for candidates who hold a Social Work degree (BA., BS., or MA), per the Minnesota Board of Social Work requirements.

Additional Requirement:

  • Fluent in the non-English language of the ethnic group served or identify with a population that is under-served in Minnesota.
  • A valid Minnesota driver's license and access to a vehicle with insurance coverage to make home and agency visits and transport clients as needed.
  • BLS/CPR Certification must be obtained within 1 month after hiring date.

Preferred Qualifications:

  • Verbal proficiency in Somali. Hmong, Lao, Vietnamese, or Spanish language.

  • Experience working with clients with mental illness.

  • Experience working in a community mental health clinic.

  • Strong time management and organizational skills.

  • Customer service knowledge and skills.

  • Ability to de-escalate mental health crisis situations.

  • Capacity to effectively communicate in difficult clinical situations and to motivate patients and others.

  • Proven ability to establish and maintain positive rapport with a diverse variety of clients and professionals.

  • Ability to set appropriate boundaries and limits for best practice.

  • Ability and willingness to provide services to children, adolescents, adults.

  • Ability to work both independently and as part of a multidisciplinary team.

  • Respect for and ability to work with a diversity of patients and staff.

  • CUHCC reserves the right to change or add duties to this position consistent with the job classification. A criminal background check is required for this position as is a negative TB test and hepatitis B vaccination series.
