SOAR (SSI/SSDI Outreach, Access and Recovery) Provides comprehensive direct support services which are structured to meet the needs of an adult population primarily composed of SMI and dual-diagnosed and/or referred persons who need assistance accessing benefits. Assists with benefit applications, obtains financial information, cooperates with and advises staff, clients and families regarding benefit issues.
Bachelor's degree in a human services related field and one year of experience working with adults experiencing serious mental illness.
Consideration will be given candidates without a 4 year degree depending on knowledge and experience in understanding eligibility of public benefits and one year of experience working with adults experiencing serious mental illness.
Certified Peer Specialist or those with previously mentioned qualifications who would be Certified Peer Specialist eligible will also be considered. Valid and current Florida Driver's License required with no more than 6 points in the past 5 years.
Must be in good physical condition with no restrictions of movement.
Must be able to apply Therapeutic Effective Aggression Management Techniques without limitations.
ESSENTIAL JOB FUNCTIONS
1.0 Assess need and directly assist with securing all entitlements for identified clients
1.1 Assist individuals applying for disability benefits using the SOAR components.
1.2 Completes interviews with clients to gather information to complete SSI/SSDI applications
1.3 Completes SOAR Medical Summary Reports for client applications.
1.4 Accompanies clients to appointments at the Social Security Administration and maintains communication with Social Security Administration (SSA) and Disability Determination (DD).
1.5 Accepts referral for assistance and assists clients with gathering all pertinent information throughout the application process, including, but not limited to assisting in obtaining past medical records, helping with appeals denials, communicating with SSA liaisons and gathering financial information using SOAR practices and guidelines.
2.0 Monitors progress of benefit process and provides assistance as required.
2.1 Develops and maintains an effective working relationship with Social Security and Disability Determination (DD).
2.2 Coordinates visits to medical doctors, psychiatrists, and other specialists to obtain evidence for eligibility.
2.3 Consults with team members on SOAR strategies.
2.4 Monitors client progress towards treatment goals in all service modalities in which the client is enrolled.
2.5 Assures that clients receive all necessary services whether provided within or outside the NHTC continuum of care.
3.0 Prepares written documentation for all services conducted.
3.1 Documentation is neat, precise, timely and accurately reflects services conducted.
4.0 Conducts case management activities for assigned clients to include assessment clients to include assessment of strengths and needs, service planning, service brokering, service and care monitoring, and client advocacy.
4.1 Case management activities are performed in accordance with Agency standards and C&F contract provisions for the delivery of case management services.
4.2 Case management activities are reflective of the services specified as being needed in the written service plan.
4.3 Case coordination is reflective of the services specified in the written service plan
5.0 Assumes primary responsibility for coordinating assigned client's care both within and outside the NHTC continuum of care.
5.1 Assures that clients who are receiving multiple services are carried out in an organized, coordinated and effective manner,
5.2 Communicates essential information regarding client care all members of the client's treatment team, both within and outside the NHTC continuum of care.
5.3 Coordinates, referrals, admissions, discharges, and transfers between service modalities both within and outside the NHTC continuum of care.
5.4 Monitors client progress towards treatment goals in all service modalities in which the client is enrolled. Actively intervenes to assure that treatment service plans remain on track and that client's utilization of service modalities remain appropriate.
5.5 Assures that clients receive all necessary services whether provided within or outside the NHTC continuum of care.
6.0 Manages assigned client cases in a fiscally responsible manner.
6.1 Monitors client's financial status, finding sources etc. and assures that limited or capacitated resources are not over-utilized.
6.2 Assures, when clinically possible that clients are enrolled in services that will maximize the resources available to them.
6.3 Continuously monitors client's status and eligibility for all forms of public and private assistance. Actively pursues enrollment of clients in programs which will provide financial assistance/payment for necessary services.
6.4 Actively seeks out alternative services/or clients who do not have financial resources/assistance to pay for services rendered by NHTC.
6.5 Delivers direct and indirect services in such a way as to maximize revenue generated by NHTC within the bounds of ethnical principles and good business practice.
6.6 Understands and assists with managed care pre-certifications and reviews to ensure needed services for the client are authorized.
7.0 Prepares written documentation for each service conducted.
7.1 Written documentation for each service conducted is performed in accordance with the Agency's standards for service documentation in the client's medical record.
7.2 Correctly completed SALs are submitted to the Patient Accounts Representative on a timely basis.
7.3 Medical records accurately reflect the nature of service activities provided, timeliness of services and outcome
8.0 Provide crisis intervention services as required.
8.1 Timely and effective crisis interventions are provided in accordance with the programs' standards and protocol for conducting this activity.
8.2 Is responsible for requests from other service modalities, both within and outside the NHTC continuum of care, for on site or telephone assistance in dealing with emergencies as well as providing essential clinical information to any professionals responding to a crisis.
