The Department of Mental Health, Central Mass Area, is seeking a qualified Human Services Coordinator I to serve as a Case Manager for adults served by the Canton Site. The primary role of the Adult Case Manager is to provide assessment, outreach, service coordination and person-centered advocacy while following the Department's philosophy of recovery and person-centered choices; utilizing strength-based goal setting through independent decision-making; establishing and maintaining a supportive relationship with individuals and families; assisting individuals in securing services; and performing all duties to reflect the highest level of respect and dignity for persons-served.

Please note: This is a full-time, 40 hour position. The schedule is 8:30 a.m. - 5:00 p.m. Monday through Friday. The days off are Saturday and Sunday.

Duties and Responsibilities (these are a general summary and not all inclusive):

  • Develops functional assessment of individual needs by the person-served, family and/or legal authorized representative (LAR) and gathering relevant written and verbal information from past and current service providers to determine the service needs.

  • Provides outreach visits and recovery-based education to the person-served and family members by developing positive relationships with them and by educating them about services to enable the individual and families to identify, accept, and obtain the necessary services.

  • Assists individuals to identify their choice of an optimal living arrangement in the community.

  • Coordinates the planning of individual service plans with the person-served and service providers, convenes meetings to modify the Individual Service Plan (ISP) when indicated; chairs the ISP meetings; writes implementation of service plans in recovery-based, person-centered language and practices.

  • Develops and maintains positive relationships with service providers and community agencies to promote cooperative planning to meet individuals' needs.

  • Facilitates periodic meetings such as conferences, risk reviews, discharge planning, program reviews, team meetings, and staff meetings to monitor service plans and make recommendations for other services that will meet individual choice and needs.

  • Maintains contact with consumers if hospitalized and assures appropriate service linkage at time of discharge.

  • Refers individuals to programs and services and facilitates their adjustment by filling out the required forms, advocating for the individual with programs, educating the individual about procedures, and by transporting when necessary to the required services to assure the implementation of service plans.

  • Maintains records by requesting all relevant information; documenting progress notes, terminating and interval summaries, meeting minutes, and by regularly updating changes in records to ensure completeness and accuracy.

  • Works with supervisor to assist in planning a coordinated community support system.

  • Complies with HIPAA regulations and confidentiality standards regarding the disclosure of PHI information.

Preferred Qualifications:

  • Demonstrated knowledge of psychosocial problems of those with mental/emotional disorders and the types and symptoms.

  • Knowledge of the principles, practices, and techniques of counseling.

  • Familiarity with the types of programs and services available to individuals living with mental illness.

  • General understanding of interviewing techniques.

  • Proficiency in Microsoft Office applications including Outlook, Word, and Excel.

Required Qualifications:

  • Strong and effective interpersonal skills, including the ability to listen well, relate to others and demonstrate sensitivity and respect for individuals.

  • Demonstrated commitment to serving a diverse population with cultural competence and sensitivity, as well as the ability to work with a wide range of constituencies with diplomacy and tact.

  • Expertise in the methods of general report writing.

Working Conditions:

This position travels to meet with clients and family members outside the office in various settings such as homes, hospitals, programs, and other accessible community settings. Candidates will need a valid driver's license and access to a reliable motor vehicle for transportation.

Applicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.

Substitutions:

I. A Bachelor's degree in a related field may substitute for two (2) years of the required (A) experience.

Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.
