Job Summary:

The information & Assistance Specialist provides information and assistance to members of the consumer populations and their families, friends, caregivers, advocates and others who ask for assistance on their behalf. This position is located in Oconto Falls.

Essential Duties & Responsibilities:

  • Provides individuals with accurate, objective and useful information.

  • Provides Referrals and/or Assistance in accessing public and private resources.

  • Documents interactions with individual customers, including the nature of the inquiry, information discussed, resources shared, decisions made and next steps. These documentations need to be completed in a timely manner.

  • Skilled in the use of the resource and client tracking databases, including how to search for services, retrieve information, and document customer contacts.

  • Have thorough knowledge of the mission, operations, and referral and confidentiality policies of the ADRC

  • Provides representation and advocacy on behalf of individuals with disabilities, when necessary and appropriate.

  • Determines needs of individuals, evaluate appropriate resources in the community, and help participants in locating resources, and actively participate in linking people to needed services.

  • Ability to recognize and appropriately respond to emergencies.

  • Ability to recognize and appropriately respond to people with special hearing, language, or cognitive needs.

  • Have general knowledge of the ADRC's client populations.

  • Become certified by the national Alliance of Information and Referral Systems (AIRS) as a Certified Information and Referral Specialist (CIRS) or Certified Information and Referral Specialist

  • Aging/Disability (CIRS-A/D) within one-year of employment.
  • Staff providing information and assistance and/or options counseling shall be knowledgeable of the Department's Dementia Care Guiding Principles and how these principles are integrated into ADRC services.

  • Completion of certification in the Long-Term Care Functional Screen within two months of hire and maintain functional screen certification throughout employment with the ADRC.

  • Meet training and practice requirements to be certified to perform Memory Screens.

  • Completes 100% time reporting and SAMs notes within timeframe required.

  • Completes all work requirements in timeframe required.

Requirements:

  • Bachelor's Degree in Social Work, Human Services, Nursing or related field.

  • Two to three years of experience working with disability populations, prior experience with Public Benefits/Entitlements desired.
