The Admissions RN is a registered nurse and an Interdisciplinary Team member whose primary responsibility is to present VITAS services and philosophy to patients and families in a manner that results in the admission of appropriate patients.


  • Participation in the information gathering, analysis, education and form completion aspects of the admissions process

  • Evaluate patients for hospice appropriateness, e.g. medical records review as requested.

  • Obtain agreement for admission of referred patients and families who meet admission criteria.

  • Analyze data collected by the LPN to facilitate/obtain admission orders

  • Build effective referral source relationships to support the high quality and responsiveness expectations associated with VITAS.

  • Maintain clinical and professional competence.

  • Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons.

  • Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient's family has concerning hospice and palliative care.


  • Knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas.

  • Ability to communicate effectively with patients and families, physicians, referral sources and colleagues.

  • Knowledge of clinical manifestations of diseases and ability to develop VITAS specific clinical knowledge.

  • Ability to use the VITAS information system and mobile technology platforms.

  • Reliable means of transportation.

  • Currently licensed as a R.N. in the state where duties are performed.

The RN Case Manager will provide leadership and support to the Hospice team, residents and families receiving Hospice care. Our Hospice team works closely with our residents and families in the end stages of life to provide fulfillment of our resident's total needs. Our friendly and compassionate nurses collaborate closely with care team staff to ensure the highest level of quality care, safety and support are provided to our residents. The RN Case Manager will supervise and oversee direct care positions such as Registered Nurses, Licensed Practical Nurses and Hospice Aides and serve as Registered Nurse Case Manager as needed. The Patient Care Coordinator will teach appropriate interventions and medications, establish, review and revise plans of care in collaboration with the interdisciplinary and support the intake and admissions process. WHAT DO WE LOOK FOR? * Associate's Degree or higher in Nursing from an accredited program * Current Registered Nurse license in the State of Wisconsin * Previous nursing experience preferred, including strong preference for (4-5) years of previous Hospice experience * Valid Wisconsin Driver's License * Valid CPR certification * Extremely high level of communication skill required; ability to tactfully have challenging conversations and provide comfort and support to residents and families. * Ability to be patient, sensitive and responsive to both the residents and their families needs. * Must be available to provide on-call coverage * Dedication to lifelong learning through attendance at in-services and classes to stay current in nursing knowledge and practices WHO IS LUTHER MANOR?
