We provide innovative and compassionate services to individuals struggling with substance use and co-occurring disorders. We are looking for dedicated, caring and compassionate people to work in our Residential/Outpatient Treatment Facilities and Community Programs.

Admissions Coordinator

Schedule per week: (Monday

  • Friday 8:30am

  • 4:30pm)

Location: Paterson, NJ

Responsibilities:

1.Maintain an admissions/discharge log. Conduct all Admissions for HWH, long-term, short-term residential treatment, Detox, IOP/OP, and Monsignor Wall Clinic. Have all intake paperwork signed including consent forms; coordinate, schedule, and act as liaison for all intake and assessment appointments; receive all phone calls regarding admissions and forward to appropriate assessor.
2.Maintain assessment log; specifically, provide a weekly report to the Program Director on clients scheduled to be assessed and/or admitted, as well as clients interviewed and reasons for acceptance or rejection; track the number of off-site assessments conducted by staff, to clearly illustrate the admissions/discharge/outreach/census process at the adolescent unit.
3.Provide a daily admissions roster list for Adolescent inpatient treatment
4.Provide a weekly report of discharges and readmission eligibility (i.e. clients who were admitted, discharged and why discharged).
5.Conduct assessments for prospective clients and Bio-psychosocial for new clients
6.Get authorizations from Performcare for Admission and Enhanced services
7.Keep close communication with Program Director to ensure all reauthorizations are submitted in a timely manner and are appropriately approved
8.Conduct orientation for new clients (explain program & rules), give initial 5-minute phone call (if needed), distribute welcome packet
9.Conduct assessments that need to be scheduled in the morning or early afternoon
10. Assist with in-house file inspections and state monitoring
11. Assist with any IA visits/investigations, etc.
12. Attend staff meetings/group supervisions, including Team Meeting (document team meeting)
13. Attend bi-weekly individual supervision or as needed
14. Assist with interventions (documentation)
15. Act as liaison with courts; correspondence for admissions status, re-admissions etc.
16. Help substitute when counselors are out of the agency
17. Ensure that treatment slots are met at capacity
18. Ensure all admission documentation is up to date and accurate
19. Help with creation and revision of policies and procedures
20. Attend any CQI sub-committee meetings as assigned as well as local meetings that will help with recruitment and outreach.
21. Complete other tasks assigned by the director
22. Supervise Admission Counselors and Office Manager in the absence of the Program Director
23. Attend Program Evaluation and Peer Review Committee
24. Follow all Electronic Health Records (EHR) policies.

Documentation:
1.Ensure that documentation of any treatment provided directly to client(s) is documented and filed in compliance with the agencys policy.

Qualifications:

Masters Degree with LCADC or working towards LCADC (LCADC plus LCSW/LPC preferred) and minimum of two years experience in the field of addictions treatment are required.

Intake Worker - Admissions Schedule per week: Monday - Friday 8:30am - 4:30pm Location: Paterson, NJ Job Specific Responsibilities and Duties * Conduct admission to HWH, long-term, Detox, Monsignor Wall Clinic, Intensive outpatient/outpatient treatment and short-term residential treatment * Review and complete all required admission documents * Provide new clients with basic information regarding our services/ programs * Complete required modules in Electronic Health Record (EHR) and NJSAMS such as DMHAS Income Eligibility, Addiction Severity Index, DSM V, and LOCI. Obtain clinical authorization for admission when required. * Forward designated documents to internal departments (e.g. medical department and welfare case manager) * Communicate with program staff regarding completed admissions * Submit all completed admissions documentation to the Director of Admissions for review in a timely manner. Ensure that files are inclusive to include all documentation as required by DMHAS and/or other credentialing entities. * Assist with managing referrals and service inquiries, as assigned, which may include the following: * Respond to and manage all messages from external and internal sources * Conduct telephone screening interviews with clients seeking treatment at Straight and Narrow. Complete screening documentation in a comprehensive, consistent, and detail-oriented manner. Request collateral documentation, when appropriate. * Follow-up on pending referrals in a timely manner, and make outreach calls to expedite admission scheduling; document accordingly * <li>Establish and maintain positive, professional interactions and relationships with referrals sources, community stakeholders, and any person or organization making contact with Straight and Narrow. * <li>Assist admissions department with other duties as assigned by Admissions Coordinator and/or Director of Admissions including but not limited to scheduling, case management, completion of preadmission consent forms, billing and filing. * <li>Assist office manager with agency roster which includes adding new admissions for the day as well as updating discharges throughout the day as needed which includes client ID number, first/last name, funding source and DOB. Assist with preparing monthly state rosters and send to DCP&P and state. * <li>Assist office manager with checking Medicaid eligibility daily and obtaining new funding sources when needed. Assist with obtaining initial authorizations with clients who have special Medicaid and or third party insurance. * <li>Assist with collecting and documenting all property received by clients including, IDS, money, Cell phones etc. Assist with managing property and IDS when counselors are signing them in and out of admissions department. * <li>Prepare and submit Medicaid and fee for service billing for the Monsignor Wall Clinic clients * <li>Participate in supervision, team meetings, and other agency committees as scheduled and/or assigned. * <li>Inform and submit documentation regarding progress towards obtaining CADC/LCADC on a quarterly basis to the Director of Admissions * <li>Follow all Electronic Health Records (EHR) policies. Qualifications: Bachelors Degree with 2 years experience in Human Services with CADC or High School Diploma or GED with 5 years experience in Human Services with CADC. If counselor intern does not have CADC he/she must be actively pursuing CADC and present supervisor with documentation demonstrating progress towards obtaining certification on a quarterly basis.
