AHC-American Health Communities is a network of 29 skilled nursing and rehab facilities. We treat patients recovering from illness, injury or surgery and provide daily care for long-term residents. Responsibilities:
The purpose of this position is to coordinate all activities of the admission process from point of referral to completion of paperwork and transfer to the Case Manager.
Description of Required Duties and Tasks:
Essential duties and responsibilities include the following.
Completes the admission process in accordance with approved policies and procedures.
Assesses patient's physical, psychological, social and environmental status; identifies appropriate interventions and initiates a plan of care based on these findings.
Records observations accurately and maintains patient's medical records in accordance with local, state and federal regulations.
Follow the established universal precautions and isolation procedures.
Participate in surveys made by authorized government agencies.
Maintain privacy and confidentiality of records, conditions, and other information relating to patients, employees and the facility.
Assist the Administrator and others in developing, implementing and updating written procedures for discharge planning.
Perform other related duties as assigned by supervisor.
Registered Nurse/ Licensed Practical Nurse Qualifications:
Currently licensed in the state of Tennessee or compact state as a Licensed Practical Nurse (LPN)
Current CPR certification
Job will require frequent lifting of objects up to 50 pounds
Supervise support staff including training, scheduling and assigning work; reviewing performance and assisting in problem-solving for staff issues
